3 THE INTENT OF THIS DOCUMENT IS TO PROVIDE GENERAL GUIDANCE. PLEASE CONSULT WITH APPROPRIATE ENTITIES REGARDING YOUR PARTICULAR SITUATION. NOTE: SOME STEPS OUTLINED IN THIS DOCUMENT MAY NOT APPLY. FOR EXAMPLE: IF YOU WILL NOT BE SELLING ALCOHOL, AN ALCOHOL LICENCE IS NOT NECESSARY; ALSO, SOME MUNICIPALITIES DO NOT REQUIRE CITY LICENSES. 1. CHECK ZONING Ensure that your site is zoned to allow a food establishment. Check with the municipality zoning office. For example, for Oklahoma City, contact City of OKC zoning phone: (405) REVIEW REGULATORY GUIDANCE Ensure that your establishment complies with required regulations to promote health and safety and to provide your business a foundation for success. State Regulations: Rules for Food Service Establishments (OAC 310:257), from OSDH, available online: You may also contact the OCCHD for additional assistance and guidance. 3. SUBMIT PLANS To construct, remodel, or convert a food, manufacturing or lodging establishment, plans must be approved by OCCHD,. Requirements for submission of plans: Plan Review Application for a Food or Lodging Establishment Plan Review Applications can be found on our website: One Set of Plans Application Fee: $425 Contact: OCCHD Building Plan Reviews at (405) Construction or remodel work must not occur until OCCHD has approved the plans. NOTE: This process allows OCCHD to assist you from the beginning and to use your resources wisely. This consultation may help you eliminate costly mistakes in the construction, remodel, or conversion of the establishment.

4 4. OBTAIN STATE SALES TAX PERMIT Contact the Oklahoma Tax Commission. You may apply online at: Allow 5 days minimum if applying online; filing in-person is recommended for expedited service. 5. REQUEST ALCOHOL LICENSE Contact the ABLE Licensing Department for the processing of all new, renewal, and change applications regarding an alcohol license. The ABLE website is a resource to help you understand the alcohol laws of Oklahoma and licensing process ACQUIRE FOOD MANAGER CERTIFICATE. For guidance and a list of accepted certificates, refer to: or contact OCCHD by phone: (405) Certificates must will be posted in the food establishment. NOTE: This certificate helps to ensure that all operators of a food establishment know and understand basic food handling principles, and is required within 45 days of health department inspection/licensing. Inspection may proceed prior to certificate being received, but ensure this important requirement is met in a timely fashion. 7. PASS HEALTH DEPARTMENT INSPECTION When you are confident your establishment meets the required standards (See Step 2), contact OCCHD and schedule an appointment for inspection. Within OKC, if a building permit was issued, phone: (405) Outside of OKC, or if a building permit not required, phone: (405) Upon passing inspection, the inspector will issue an Application for Food Establishment License (ODH Form 828). If in OKC, the inspector will also issue an Application for the OKC Food Establishment License. 8. OBTAIN STATE LICENSE Must be paid within 4 business days from day of licensing inspection. Mail or deliver in person the white copy of the ODH Form 828 to OSDH.

OBTAIN CITY LICENSE If establishment is in OKC, hand---carry the following required documents to the City of OKC: Application for the OKC Food Establishment License Copy of Health Department Inspection Sales Tax Permit Certificate of Incorporation, or LLC (if applicable) 10. RECEIVE AND DISPLAY LICENSES As licenses are received, place them in a visible location inside your food establishment. NOTE: Licenses are not transferable from one individual to another. If you change ownership or lease your food establishment to another person, they must obtain their own licenses. 11. RENEW LICENSES ANNUALLY Ensure that you renew your licenses every year in a timely manner. For establishments in OKC, MAIL OR HAND DELIVER IN PERSON-CARRY THE FOLLOWING REQUIRED DOCUMENTS TO THE CITY OF OKC: Current Oklahoma Sales Tax Permit Inspection from OCCHD for last licensed year Photo ID General application to renew the license: Immigration affidavit (if applicable or not on file) Payment ALCOHOL LICENSE RENEWAL Contact ABLE. STATE FOOD ESTABLISHMENT LICENSE RENEWAL OSDH will send you a renewal notice in the pail. Pay the renewal before the license expires.
