1 Interested in selling your produce and/or value-added agricultural-based products at the Madison County Farmers' Market (MCFM)? The MCFM accepts applications from any and all Kentucky farmers and producers. All new and returning vendors must fill out the primary information and application. All applications must be submitted with applicable copies of vendors' City of Richmond business license, certificate of general liability, KDA Produce Best Practices Training (PBPT) sampling certificate; KDA Cooked and/or Processed Sampling Permit; All Samples Sampling Certificate; Home-Based Processor Permit; Home-Based Micorprocessor Permit; and/or Retail / Commercial Manufacturer Licenses. IMPORTANT NOTE: Vendors will be prohibited from participating in the Madison County Farmers Market if any of the required documentation is missing from their application. All required documentation must be displayed or readily available each market day at the vendor's station. The purpose of Madison County Farmers Market (MCFM) is to preserve Kentucky farmland and promote local food sourcing and sustainable agriculture. The Market provides an excellent platform by which growers may sell their fruits, vegetables and other farm products directly to consumers. The MCFM is operated in the belief that the open marketplace provides the most beneficial means of exchange between farmers and residents. All applications must be received by April 9, 2019, for consideration. Applications received after April 9th will considered only on an application-by-application basis, even for returning vendors. In order to be considered for the 2019 MCFM, all vendors must be in good standing; if you are not in good standing and wish to apply for the 2019 Market, please contact us to settle your account. 1

This year as in previous years, ALL MCFM vendors must submit copies of the following documents with their application as applicable to your products: 2019 MCFM Vendor Application and Payment - required; City of Richmond Business License(s) - required; Certificate of Completion for the GAP (Good Agricultural Practices) or PBPT (Produce Best Practice Training) - required Cooked and/or Processed Sampling Certificate - required for any vendor offering sampling; All Samples Sampling Certificate - required for any vendor offering sampling of fresh produce, or non-cooked and/or processed foods; Home-Based Microprocessor Certificate - required for vendors selling processed non-fruit (vegetable or herb) or sugarless jams/jellies, acid foods, acidified food products and/or low-acid canned foods including tomatoes, green beans, salsa, barbecue sauce, pickles, chutney and similar products; Commercial Retail Packaged Food Permit/License/Registration/Certificate(s) - required for any vendor selling commercial-based processed foods, including all dairy products; Egg Retail License - required if selling more than 60 dozen eggs per week; Nursery Stock - if plants being sold can be placed outdoors, survive through the winter and then start growing the following spring, a nursery or nursery dealer license will be required. Licenses will not be required if the seller is only selling annual flowers or annual herbs, annuals, cut flowers, garden mums or vegetable transplants; Pet Foods and Treats - vendors selling dog biscuits or other specialty pet treats are required to register with the Division of Regulatory Services and meet specific labeling requirements of the law; Lip Balms and Cosmetics - Lip balms, lotions, sugar/salt scrubs and any other product applied to the body (cosmetics) may only be manufactured by a permitted facility; Wine Sales Licensing - required copy of the wineries' Kentucky ABC license; NO tasting or sampling permitted at the MCFM
