Renewal of membership is available upon invitation from the Chardonnay Golf Club. Membership to the Chardonnay Golf Club is non-transferable and non-refundable with the following exception. The club agrees to refund a pro-rata amount for any pre-payment of membership fees, less five percent (5%) administration fee within thirty (30) days after receipt of written request for refund due to death or permanent medical disability, accompanied by, notice of death or proof of a permanent medical disability. The obligation by the club to repay any pro-rata amount due to pre-payment of the membership shall be subject to a set-off of all amounts, which the member owes the club. Early termination of your membership, other than for the reasons stated above, will result in an early termination penalty, as follows: Payment of one months dues for every partial and/or full year remaining on the contract For pre-paid memberships, any approved pro-rata refund by the club for early termination shall be subject to and setoff by these early termination penalty fees, along with any other amounts, which the member owes the club. Membership is contingent upon approval by the Chardonnay Golf Club, which approval shall be at its sole discretion. Upon signing this application, I authorize the disclosure and release of information requested by the club for investigation of my qualifications for membership, including without limitation, my credit history. I hereby acknowledge that the use of the Chardonnay Golf Club facilities and any privilege or service incident to membership in the Chardonnay Golf Club is undertaken with knowledge of risk of possible injury. I hereby accept any and all risk of injury to myself, my guests and my family sustained while using the club facilities or involved in any event or activity incident to membership in the Chardonnay Golf Club. In accepting the risk of injury, I understand that I am relieving the Chardonnay Golf Club and its successors and assigns, and their directors, officers, partners, shareholders, employees, agents and affiliates from any and all loss, costs, claims, injury, damages or liability sustained or incurred by me, my guests, and my family resulting from or arising out of any conduct or event connected with membership in the Chardonnay Golf Club and use of any of the club facilities. I hereby agree to be bound by the terms and conditions of the rules and regulations of the Chardonnay Golf Club. The Chardonnay Golf Club may amend the rules and regulations from time to time. I further acknowledge that I am not relying on any oral representation in acquiring a membership in the Chardonnay Golf Club.
