1 2018 FOOD VENDOR APPLICATION AND CONTRACT July 27-29, 2018 Application Deadline- MAY 10, 2018 VENDORS WILL BE NOTIFIED OF ACCEPTANCE BY MAY 18, 2018 Mail Applications to: Craft Cooperative of the NW 1916 Pike Place #146 Seattle WA Thank you for your interest in the Bellevue Festival of the Arts, we are pleased to announce this year s event will be July 27-29th, The Bellevue Festival of the Arts features approximately 170 fine art and craft vendors, a variety of food concessions, and musical entertainment and is free to the public. We are located just north of Bellevue Square in the former Cost Plus World Market lot on NE 8th and Bellevue Way. This year marks the 34th year consecutive show produced and sponsored by the Craft Cooperative of the Northwest, a nonprofit group of artists founded in The music stage is located in the food court area. The festival occurs concurrently with the CIty of Bellevue Arts Fair Weekend with the nearby Bellevue Art Museum ArtsFair and the Sixth Street Fair. Approximately 50,000 to 60,000 people visit our food court each year. All applicants will be notified by on May 18th, If you are accepted, we must receive your county food permit (or permit application), booth / electric payment check, copy of insurance policy, and separate cleaning deposit check by June 10th, Questions? us! bellevuefest.org FOOD VENDOR TIMELINE: May 10, APPLICATIONS CLOSE: All applications must be completed and postmarked by this date to be considered. No incomplete applications or applications via will be accepted. Mail completed application to the address at the top of this page. May 18th, Notifications: All applicants will receive an indicating their acceptance status by this date. Due to limited space and other considerations, not all applicants will be accepted. June 10th, 2018 PAYMENTS & PERMITS DUE: For all accepted food vendors, mail all 4 items below, postmarked no later than June 10th, 2018 to secure your booth. Partial refunds may be considered up to June 20th, no refund of fees will be given consideration after this date. Make all checks out to CCNW ( ) booth and utilities payment (1 check w combined total of booth fees and electrical needs) ( ) cleaning deposit (1 check, separate from fees, returned after load-out inspection) ( ) certificate of insurance ( ) copy of county food permit or permit application *Remember: The Health Department requires you to apply for your Temporary Food Permit at least two weeks prior to the event! July 26th, Load In and Set Up: All food vendors that are not food trucks are required to set-up Thursday night before the Festival, beginning at 8pm at our staging lot at the Bellevue Christian School (BCS). See below under LOAD-IN for more information. Trucks may drive on Friday morning and must arrive no later than 8AM. July 27-29th, Bellevue Festival of the Arts: All food vendors must remain open

2 during all festival hours each day. Restocking can be done before and after hours with vendor ID for security. WATER NEEDS: BOOTH BILL: 10x10 Booth space fee = $1350 (flat fee, no commission taken) $ ELECTRIC 120 volt duplex outlet, 10 amps= $90 and/or 120 volt 4-plex outlet, 20 amps= $110 $ CLEANING DEPOSIT: $ (REFUNDABLE)* $ (send as a separate check from booth fee, check is returned after load-out site inspection) TOTAL DUE JUNE 10, 2018 TO CCNW UPON ACCEPTANCE: $ ( do not send checks with application, notifications sent May 18, 2018) *REFUNDABLE Cleaning deposit $300. Please enclose a separate check for this which we will return to you within 7 business days after the end of the fair when cleanliness standards have been met. Food Vendors are responsible for cleaning their area before leaving and will bear responsibility for any extra cleaning charges. Food Vendors must bring their own cleaning supplies such as brooms, degreaser, etc for keeping their space clean throughout the weekend. Food Vendors must keep their booth and surrounding area clean and organized during all open hours, must maintain all their equipment to Health Department standards and must keep all their materials within the boundaries of their booth. Festival Quality Control Staff and the Bellevue Health Department will be present at the

3 festival to ensure a high quality event and to help with any issues that may arise Bellevue Festival of the Arts Food Vendor Rules & Regulations: (Read and initial at end) Show Hours: All food vendors must remain open during all festival hours each day. Restocking can be done before and after hours with vendor ID for security. Festival hours are: Friday & Saturday: 10am-8pm, Sunday 10am-6pm COUNTY FOOD PERMITS : Vendors are responsible for submitting a Temporary Food Service Application to the Bellevue Health Center at SE Eastgate Way or downtown Seattle. The application is due at the office at least 14 days prior to the event. Notice of that application along with your insurance certificate is due to CCNW no later than June 10, All workers must have food handler permits before working at the event. Permits can be obtained from any district health office. Call for more information. STATE BUSINESS LICENSE : If your business does not have a valid Washington State Tax ID#, contact the Department of Revenue. SPECIAL EVENTS TAX : The City of Bellevue assesses $5 a day from each vendor which is included in our fee, and we pass it on to the City of Bellevue. INSURANCE : Food Vendors shall obtain primary insurance, naming CCNW as an additional insured, with coverage in the amounts of $1,000,000 aggregate bodily injury and property damage (including personal injury and contractual liability) and premises medical coverage. Insurance certificates must show CCNW as an additional insured and list Bellevue Festival of the Arts, July 27-30th as description of operation and location. Coverage must include Thursday set up. Any insurance on the premises which has been purchased by the Lessor (and CCNW) will be considered between the parties hereto as excess coverage over the insurance provided by the food vendor. The Description of Operations is: Bellevue Festival of the Arts NE 8th St Bellevue WA July 26-29, 2018 (or July 27-29, 2018 for Friday drive up food truck vendors only) The Certificate Holder is: Craft Cooperative of the NW 1916 Pike Place #146 Seattle WA FIRE EXTINGUISHER : The City of Bellevue fire department requires you to have a portable fire extinguisher of sodium bicarbonate, potassium bicarbonate, or the class K type in each food booth. CCNW does not provide fire extinguishers. The Fire Department does an inspection early Friday morning to ensure the fire extinguishers are unexpired and functional. BEVERAGES: Volunteers from our community partner, Bellevue Christian School, will be selling carbonated beverages and water EXCLUSIVELY at our food court. No carbonated beverages or bottled water may be sold by food vendors. However, specialty beverages with carbonation will be considered for sale by food vendors. Thank you for your understanding of our effort to help the BCS students raise money for their school choir. FOOD ITEMS : Food Vendors may only sell the type of food listed on their application. No substitutions are allowed unless cleared through the CCNW office 15 days prior to the event. PARKING : Food Vendors will abide by loading and unloading procedures as prescribed and provided by CCNW. See below under LOAD-IN for more information. No parking, loading, unloading, set-up or tear-down on the streets or sidewalks at any time. Vendors further agrees to park only in authorized areas as assigned by CCNW. Parking is very limited near the Festival site. We will have morning and evening shuttle service to our staging lot at the Bellevue Christian School. Shuttle service hours of operation will be included in info packet for vendors along with parking permits and vendor ID s. Parking for food booth staff members is available at nearby parking garages for a fee or at our staging lot at the Bellevue Christian School for free. Food booth staff may use the shuttle bus service if they choose to park at the Bellevue Christian School. LOAD-IN: Food Vendors must be available to set up Thursday night, July 26, 2018 We will provide you with driving directions, parking permits, and vendor ID prior to Load In The load-in and set-up for the festival occurs the night before the show, Thursday July 26th. Our fair site is located in a bustling, dense, urban downtown setting, and together we to build a mini art city within it! Our load in process is designed to meet City codes and maximize Food Vendor convenience and space.food truck vendors

4 may load in on Friday morning, arriving no later than 8am for check in and prep. We have a large helpful staff to manage the process and provide assistance. A staging area at the Bellevue Christian School is used for artist and food vendor check-in and check-out to ensure the most efficient use of the fair site. All food vendors will receive a load-in packet with a parking permit and specific instructions when they arrive at the staging lot at the Bellevue Christian School. The next two paragraphs are an overview of the process Food Vendors must follow during load-in if setting up on Thursday night. 1. Food Vendors must arrive at the staging lot at their pre-assigned time, and check-in with the Blue Section leader to receive a welcome packet and load-in parking passes. Then they wait until they are called to drive down to the festival site. Food Vendors can expect a short wait between check-in and festival site entry, and they will be directed by their section leader. Snacks and high-fives will be provided by our staff and volunteers. Pre-assigned times are staggered as are times when food vendors are called down to the festival site. The staggering of these times is crucial for keeping traffic flowing and making for an easy load-in. 2. The Blue Section leader will direct Food Vendors to a specific entry point to the festival site and give them a stamp that they use to gain entry onto the festival site. The Food Vendor then drives to the entry point and is directed by festival staff to their booth space. Booth set up can last until after dark, so please bring any lighting you might need. There are overhead lot lights, but flashlights or headlamps are encouraged. The parking lot to the west of booths F 7-9 is off limits. It is not included in our lease and is not to be used at any time for storage, parking, etc. SECURITY: Booths may be left up overnight at the sole risk and responsibility of the Food Vendor. Security guards patrol the site between 8pm and 10am Thursday, Friday, and Saturday evenings. All products should be removed from booths that are left up overnight EXHIBITION STANDARDS: a. Food Vendor agrees to set up booth within restricted boundaries set forth by CCNW. Equipment, vehicles, storage, chairs and portable display items may not extend beyond booth boundaries. b. Food Vendor must provide booth and props for display and cover in case of bad weather. c. No poly tarps may be used in the booth. No rusted or well-worn galvanized pipes - pipes must be covered with fabric. d. Unpainted materials (pegboard, plywood, lumber, etc.) are not permitted. Booths must be pre-built and assembled on site. e. Booths must have sides and a back available for night closure and booth boundaries. (Initial) I have read and understand the 2018 Bellevue Festival of the Arts Rules and Regulations Food Vendor Legal Agreement (signature required) It is understood that the Bellevue Festival of the Arts is under the sponsorship of the Craft Cooperative of the Northwest. The undersigned agrees to hold said agents harmless from and against any and all liability the right to be in the show and which may arise out of the acts and/or negligence of the Food Vendor or other persons acting on the behalf of the Food Vendor. Food Vendor further agrees to hold the Cooperative, its officers, directors and employees harmless from and against any and all products liability claims which may arise out of the sale of products by the Food Vendor in the show. Food Vendor agrees to maintain all current permits and insurance requirements and is understood violation of these terms will result in immediate removal from the Bellevue Festival of the Arts with no refund of fees. Food Vendors have understood that all payments, deposits, fees, insurance, permits must be postmarked by June 10th or the Food Vendor forfeits their space. All efforts will be made to assist Food Vendors, however the responsibility to meet all deadlines lies solely with the Food Vendors named in this application. Food Vendors must have current insurance and all employee and business health department paperwork available at the festival and must adhere to any Health Department, staff, or fire department inspection. Food Vendor has read and understood the above. Food Vendor has also read and understood the above show rules and regulations and agrees to abide by them. Violation of any of all of these rules and regulations could result in Food Vendor being asked to leave the show. I hereby release and hold harmless the Craft Cooperative of the Northwest from any and all personal liability including, but not limited to, theft, personal injury, bodily injury, strike, public enemy or act of God and to indemnify it for any damages arising from my conduct at the event.
