Craft Service Provider Crew Deal Memo Rider Reference is made to the Crew Deal Memo agreement dated as of, 2013 ("Agreement") between Columbia Pictures Industries, Inc. ("Employer") and ("Employee") in connection with the motion picture entitled "London Calling" (the "Picture"). Employee acknowledges the following: 1. Employee shall handle and prepare food in accordance with the California Uniform Retail Food Facilities Law, or the equivalent thereof in any location outside of California. 2. Employee shall abide by the applicable locality s health and labor laws. 3. Employee shall carry appropriate food handling certification in accordance with the California Uniform Retail Food Facilities Law, or the equivalent thereof in any location outside of California, and shall provide evidence of such certification to Employer upon request. 4. Employee shall provide to Employer evidence of appropriate personal Auto Liability Insurance coverage if Employee s vehicle is not classified as a commercial vehicle. 5. Employee shall prepare food in an appropriate venue in accordance with the California Uniform Retail Food Facilities Law, or the equivalent thereof, i.e., in an appropriate kitchen facility that is not a residential kitchen.
