Our venue rentals include plates, silverware, event set-up and tear-down. Linen service is available upon request. Dance floor is available upon request for an additional $150 fee. Bridal Party Risers are available upon request for an additional $100 fee. Both venues have a four-hour time limit. For events that last longer than four hours, the host will be charged an additional $150 per hour. Additional hours must be decided 30 days prior to the event due to scheduling and staffing needs. Catering Selections: Must be chosen at least 30 days in advance to ensure proper supplies and services needed for the function. If choices have not been made during this time frame, the Catering Director will make the selections on your behalf. Please ask your guests if they have any special dietary needs or limitations before your function. Last minute menu changes will be charged in addition to your original guaranteed cost. No outside food or beverage may be brought onto Ashland Golf Club property, with the exception of wedding cake and desserts. The event host will be charged a $50 convenience fee for the storage, handling and cutting of the wedding or event cake. For your safety, leftover food items will not be packaged to go. Delay: Ashland Golf Club is not responsible for the quality of food if the event or function is delayed more then 30 minutes after the agreed time. Attendance Counts: Ashland Golf Club reserves the right to know the final number of anticipated guests 30 days prior to the function. Decorations, Displays and Other Materials: Must not defy city codes or fire regulations. We ask that you kindly refrain from taping or tacking on the walls, ceilings or light

fixtures without prior approval. Additional Clean up fees will be issued for facilities left in poor condition after a function. The curtains enclosing the pavilion are to be handled by Ashland Golf Club staff ONLY. There will be an additional charge to the final bill for individual guests attempting to open or close the curtains. All decorations for wedding ceremonies and receptions are to be provided by the event host. Assigned Seating: If the party desires assigned seating, you must provide place cards for your guests including table numbers and meal selections. Cards must be in alphabetical order, accompanied by an approved floor plan. Entertainment: Providing optional entertainment is the sole responsibility of the guest, and must be approved by Ashland Golf Club. Entertainment is required to stay within certain volume limits and avoid disturbing other functions. Set-ups: Ashland Golf Club is responsible for all function set-ups. Special requests for linens will be at the client s expense. Dance floor available upon request, additional $150 fee. Risers available upon request, additional $100 fee. Wedding receptions may come to decorate the evening before, unless otherwise booked for another event. Guests decorating the evening before will be charged $150 per hour (with a 2 hour minimum) to guarantee the event centers availability. Alcohol Policy: Absolutely no outside alcohol will be permitted on Ashland Golf Club premises. If alcohol is found, we will ask your guest to remove it or they will be asked to leave. Our bartenders reserve the right to refuse service to those they deem as overly intoxicated. Wedding Receptions with alcohol are required to have a police officer present at the expense of the host ($42/hour through the Ashland Police Department). Ashland Golf Club is responsible for scheduling officers for events. Event Hosts will not be permitted to schedule their own security. Bartenders will perform last call 15 minutes before your official function end time. Event Center Deposits: A rental fee of $500 is required to be paid in full at the time of the reservation confirmation. Deposits are non-refundable if an event or function is cancelled. No event will be confirmed until the agreement has been signed and the required deposit has been received. Events in the pavilion require a $1,500 minimum. Those who do not meet the $1,500 minimum will be charged the difference on the final invoice. Event hosts will be charged a $100 clean up fee. Tent Rental Deposits: A rental fee of $300 is required to be paid in full at the time of the reservation confirmation. Deposits are non-refundable if an event or function is cancelled. No event will be confirmed until the agreement has been signed and the required deposit has been received. Events in the tent require a $1,000 minimum. Those who do not meet the $1,000 minimum will be charged the difference on the final invoice. Event hosts will be charged a $100 clean up fee.

Wedding Ceremonies: Those who wish to conduct wedding ceremonies on Ashland Golf Club Property will be charged a $300 ceremony fee. This $300 includes the chair rental and set up/tear down. Damages: The event host will be responsible for any damages or loss to Ashland Golf Club property, equipment, decorations or fixtures due to the actions of the attendees of the event. Payments: The cost of the event/function will be paid in full immediately following the event. Payments must be made prior or on the day of. No event will be permitted to pay at a later date. Obligations: All obligations are contingent upon Ashland Golf Club s ability to perform and are subject to travel restrictions, acts of God, or any other circumstance beyond the control of Ashland Golf Club, which may prevent it from performing its obligations. The client executing this agreement expresses that he or she is over eighteen years of age, and is authorized to execute this agreement on behalf of the organization. Ashland Golf Club is not responsible for damaged, lost or stolen items prior to, during or following the function. *All services, fees and menus are subject to a 20% Service Charge and 7% Ohio Sales Tax Charge. Leftover Food Waiver and Release from Liability By signing this waiver, I, release Ashland Golf Club from any liability with regard to possible spoilage or food-bourne illness from leftover food removed from the event site from the event on 2017. I am aware that the food has been removed from the kitchen approximately 2 hours prior to the event and that the food has been on display at the buffet for the duration of the event. I am also indicating that I accept full responsibility for removal and safe storage of the leftover food. Signature Date
