Included is the information you need to apply as a food vendor. Please note our Important Art Fair Requirements. Refusal or inability to comply with these requirements disqualifies you for consideration. The attached application serves for the Michigan Union Food Court as well as the Food at the Fountain food court at the Michigan League. For those of you applying for more than one location, please submit an application and a deposit for each location. IMPORTANT ART FAIR REQUIREMENTS 1. TEMPORARY FOOD LICENSE: All Food Vendors are required to obtain a Temporary Food License from the Washtenaw County Health Department. The Health Department will have meeting dates and times scheduled during the months of June and July for local vendors. Please call them at (734) 222-3800 to make arrangements. For out of state vendors wishing to avoid the late charge, please mail a list of your menu items along with your check or money order to: Washtenaw County Environmental Health Attn: Food Service 705 North Zeeb Rd P.O. Box 8645 Ann Arbor, MI 48107-8645 All forms as well as safety tips and checklists are available at: http://www.ewashtenaw.org/government/departments/environmental_health/food_safety/eh_tempfoodinfo.html All paperwork involving the Health Department must be sent to them directly (i.e. Health Department applications and fees). The University Unions have arranged for one Health Department meeting to be held on-site for any vendors unable to make prior arrangements. This is one component of a mandatory safety meeting conducted in conjunction with the fire department, health department and University Union staff. MANDATORY SAFETY MEETING FOR VENDORS Tuesday, July 16, 2013 Wolverine Room, Michigan Union 1 st floor 2:00pm 2. HEALTH DEPARTMENT COMPLIANCE: All food preparation areas must be screened in. Vendors are required to provide their own back flow preventor and hose. The hose must be Health Department approved. Michigan s Food Law 2000 prohibits bare hand contact with ready-to-eat foods. Suitable utensils such as deli tissue, spatulas, tongs, single use gloves, or dispensing equipment are permissible alternatives. If raw meat, poultry or fish are being handled on site, a designated hand washing station is required. This requirement can be met by having: potable water in a dispensing container, anti-bacterial hand soap, a catch basin for used water, clean paper towels and a garbage container. Individual hand wipes are acceptable when no raw meat, poultry or fish are handled on site. Cold foods must be held at 41 F or less. Hot foods must be held at 140 F or above. Raw meats, poultry, fish or other potentially hazardous foods MUST be held under mechanical refrigeration.

Frozen raw meats, poultry, and fish can be thawed in the following ways: under mechanical refrigeration at 41 F or less, fully immersed under running potable water at 70 F or less, or as part of the cooking process. Raw meats, poultry, fish or other potentially hazardous foods CANNOT BE THAWED OR STORED in ice chests. No smoking or eating is allowed in food booths or trailers. All employees are expected to maintain good personal hygiene and be in good health. 3. FIRE SAFETY COMPLIANCE: Propane users will be required to: Use cylinder / tanks of 100 pounds or less. Limit back-up to one per live or active cylinder. Have supplies of propane fuel delivered on an as needed basis. Follow AGA/NFPA codes regarding portable propane tanks. SECURE ALL TANKS! Keep propane tanks outside of booth or trailer. All propane cylinders must be removed no later than 10:00am Sunday, July 21, 2013. A non-refundable charge of $25.00 per tank will be assessed after 10:00am (this will be deducted from your cleaning refund.) Charcoal grilling is prohibited. All booths must be fire resistant. Materials must be labeled or have a letter on site from the manufacturer attesting to its fire resistance. Wood framing must be protected with a non-flammable coating (i.e. latex paint). Extension cords are the responsibility of the vendor and must be #12-3 wire or larger. Multi-tap extension cords are not permitted unless rated for outdoor use (GFCI). A 2A-10BC fire extinguisher is required (5lb. ABC) with a servicing tag, indicating inspection and/or service within the last 12 months. A Class K fire extinguisher is required if you are using vegetable or animal oils or fats. If manufactured after 1/1/02, shall not be equipped with extended wand-type discharge devices. 4. TENT WEIGHTS: The fire Marshall will be enforcing the requirement, set forth in NFPA 102 8-3, which requires that each tent post be sufficiently weighted to resist a minimum wind pressure of 20lmf/ft2 (0.06 kpa) of projected area of the tent. The Fire Marshall also strongly discourages the use of Easy-up/Pop-up tents due to the lack of structural strength. 5. LOADING / UNLOADING & PARKING: No vehicles may remain on the Art Fair site. The loading/unloading pass must be displayed on the dash of your vehicle and must be clearly visible. Violators will be towed at the owners expense. Parking will be available again through Republic Parking at spaces within a few blocks of our sites. 6. TRASH AND RECYCLING: Each booth/trailer must provide a trash receptacle next to their customer service area. The vendor is responsible for maintaining these trash receptacles and the area surrounding their booth/trailer. Recycling of corrugated cardboard, glass, tin, and aluminum will be available as well as a grease barrel. All gray water (and only gray water) must be dumped at the designated sewer drain.

MENU SELECTIONS Vendor Name The Michigan Union Food Court has been a part of the Art Fair since 1985 and is in the vicinity of the Ann Arbor Summer Art Fair. All menu items will be considered. Food at the Fountain became part of the Art Fair in 2004 in response to the new location for the Ann Arbor Street Fair. Organizers for that fair have requested that this food court incorporate unique, high end food items with a focus on lighter fare. We strongly recommend that you minimize your number of menu offerings. This will streamline your food preparation, make the customer s decision a bit quicker and help reduce Health Department concerns. Remember that Ann Arbor requires screened in prep areas. List all food and beverage items to be sold with accompanying prices. An exact listing of food items is required by the Health Department. This list CANNOT be altered without written permission from University Unions Food Courts. Menu Item Price Your approved menu items and prices must be clearly displayed at your location.

ELECTRICAL APPLICATION Vendor Name You must order all electrical needed on this form. We may not be able to accommodate requests for additional electrical circuits upon your arrival. If your equipment requires more than 208 volts, we recommend that you bring a power- boosting transformer. A hookup fee (based on time) will apply if our electrician has to perform the wiring of any transformers. Please indicate how many 120-volt circuits you require. If you have more than 2 freezers or refrigerators, it is strongly recommended that you order at least two 120 volt circuits.
