The Foundation Room, located on the 63rd Floor of Mandalay Bay Resort & Casino, offers one of the city s most breathtaking dining locations. The Foundation Room chefs will be offering a five course, prix-fixe menu beginning at 6pm. Dining includes a 50% discount for access to the Foundation Room Lounge. Please see below for Dinner Package description and pricing. Foundation Room Members will receive complimentary champagne for themselves and their guests throughout dinner. Foundation Room members receive a 10% discount on all packages. FOUNDATION ROOM RAMA PACKAGE SIX O CLOCK RESERVATIONS $175 PER PERSON PLUS TAX & GRATUITY Included in this experience is a five course menu, a complimentary glass of champagne and a 50% discount for access to the Foundation Room Lounge after you have finished your dining experience. LOUNGE PACKAGES The Foundation Room Lounge will open on New Year s Eve at 9 o clock for admission and Bottle Service Packages. There will be no food service in the Lounge. Tax and gratuity will be added to package pricing. Foundation Room members receive a 10% discount. BOTTLE SERVICE AND TABLE MINIMUMS Bottle Service Packages and Table Minimums are available to accommodate varied party sizes. Additional guests may be added to any table reservation for the pricing structure listed below and all package prices include complimentary admission. All seating in the Lounge, Private Rooms, Dining Side Patio and Lounge Side Patio are reserved for this service. Packages for access will be available for pick up beginning at 1pm. We recommend picking up packages between the hours of 1pm and 7pm at the Foundation Room. Mandalay Bay is subject to change the door closure due to Clark County policies, so please arrive as early as possible. KĀMADEVA PACKAGE $8,000 Includes the very finest in table service the Foundation Room has to offer accompanied with the premier table location in the venue. This package will accommodate up to 12 guests, 2 Magnums of premium Champagne and your choice of 2 Magnums of premium spirits. Additional people may be added to this package for $150 each. SUTRA PACKAGE $5,000 Includes either the Buddha or Ganesh prayer room or highly desirable tables in main lounge and outdoor lounge patio, and the very finest in table service the Foundation Room has to offer. This package will accommodate up to 10 guests, two bottles of premium Champagne and your choice of three additional premium spirits. Additional people can be added to this package for $150 each. TANTRA PACKAGE $4,000 Includes a coveted table on our outdoor patio directly in front of our balcony with an unobstructed view of the Las Vegas strip. This package will accommodate up to 6 guests, two bottles of premium Champagne and your choice of two additional premium spirits. KAMA PACKAGE $3,000 Includes seating in our outdoor lounge patio or indoor lounge. This package will accommodate up to 6 guests, two bottles of premium Champagne and one choice of premium spirit. HOTEI PACKAGE $2,000 Includes seating in our media room or indoor lounge. This package will accommodate up to 4 guests, one bottle of premium Champagne and one bottle of premium spirit.

VIP ADMISSION $300 PER PERSON Inclusive of admission, tax, and open bar with doors opening at 9 pm and is valid until venue closes. Please note, general admission does not include a table, seating, or fireworks viewing. We have limited space on the patios. Entrance to the patios without a patio table reservation is strictly based on first come first serve basis. Open bar includes non-alcoholic beverages, imported and domestic beers & well liquors and champagne toast at midnight. GENERAL ADMISSION $150 PER PERSON Inclusive of admission, tax, and open bar with doors opening at 9pm. Open bar valid from 9pm to 11pm. Please note, general admission does not include a table, seating, or fireworks viewing. We have limited space on the patios. Entrance to the patios without a patio table reservation is strictly based on first come first serve basis, package includes a champagne toast at midnight. LOCATION Foundation Room is located atop Mandalay Bay at 3950 S. Las Vegas Blvd, Las Vegas, Nevada 89119. DEPOSIT POLICY In order to reserve a space, we must be in receipt of a credit card authorization form and a signed agreement from you upon receipt of this package. If a credit card is used as a deposit, the credit card will be held and charged the balance of your dinner or lounge table on New Years Eve. Your dinner/lounge table is not confirmed until the full deposit and a signed agreement is received by this office and is subject to cancellation without notice or consideration. CANCELLATION POLICY Cancellation of your event within 72 hours of the scheduled time will result in forfeiture of your reservation. 100% applicable charges will be charged. If you have supplied a credit card for your deposit, the deposit amount will be applied to your credit card upon confirmation. All deposits become the sole property of the restaurant and will be forfeited upon cancellation for goods and services rendered in good faith. CHECK IN POLICY Dinner reservations will be held no later than 15 minutes past the specified time. Foundation Room Lounge reservations must check in for their table no later than 10:30 pm. Foundation Room lounge will be open at 9:00pm on Sunday, December 31, 2017. SPECIAL DIETARY REQUESTS All special dietary requests must be addressed prior to the completion of this contract. Foundation Room will do everything reasonable and customary to accommodate dietary requests and needs. DRESS CODE POLICY The Dress Code Policy is in place to maintain the sophistication of a selective club for professional and personal entertaining while embracing attire that does fit within the genre we promote for our nightlife. To achieve that, all attire is welcome with the exception of shorts, sandals, hats, t-shirts, sweatshirts, sports apparel, tank tops, sneakers and athletic shoes. Design and logo collared shirts, dress shoes, jeans and blazers are encouraged as long as the design is tactful and any markings are intentional from the designer.

MISCELLANEOUS Must be 21 or older to attend. The host is responsible for any and all damage to the premises incurred by his/ her guests. Foundation room does not allow guests to bring outside food and beverage, per policy, into the restaurant without expressed written consent of the Director of Foundation Room Operations. This includes, but is not limited to, wine, champagne, liquor, food prepared or in its raw state, desserts, cakes, etc. Foundation Room and House of Blues reserve the right to inspect and control all private functions. Foundation Room and House of Blues cannot assume responsibility for personal property or equipment brought into the function area. Personal effects and equipment should be removed from the premises at the end of the event. TAX & GRATUITY In addition to the table package pricing, tax and gratuity will be applied to all purchased table packages. AUTHORIZATION I have read the above agreement, restaurant policies and procedures printed in this agreement. I agree to the terms and conditions mentioned above. This booking will remain tentative and subject to cancellation by the restaurant until this contract is signed by the organization (you) and received by the restaurant (us) along with authorization of prepayment. I further understand that if I choose to cancel my event within a period of 72 hours prior to the scheduled time I will forfeit my prepayment to the restaurant for goods and services rendered in good faith. Furthermore, I do not hold the restaurant liable for the cancellation of my event either by the restaurant or myself. I release and indemnify Foundation Room and its staff for any and all loses due to a cancellation on my part. I understand and agree to the terms and conditions of this agreement and enter into it in good faith with Foundation Room. I understand and accept that I am liable for the total cost of the reservation booked that I have agreed to, and that I am liable for full payment of the guarantee, as well as any and all charges above the guarantee, as stated in the guarantee policy. I understand that I am liable for any and all taxes and gratuities associated with this event. By the card holder signing, you agree to all terms listed in the contract. Host is required to sign only as an acknowledgement of event. FIREWORKS DISCLAIMER Please note that general admission does not guarantee fireworks viewing.
