SFA thoroughly understands federal and state regulations. Sponsor was very timely; sent and completed all off-site documentation well before due date. This institution is an equal opportunity provider.

2 Site - Level Commendations: BACON ELEMENTARY (0490) All staff were very friendly and did a great job making sure all kids had reimbursable meals. Helen runs a well-organized, clean and efficient kitchen. The salad bar had great variety; the students were very excited about the fresh berries! Site - Level Commendations: FOSSIL RIDGE HIGH SCHOOL (3105) All staff did a great job identifying reimbursable meals and encouraged students to select fruit/vegetable in order to receive a reimbursable meal. This was especially impressive given the number of kids going through the line in a short amount of time at lunch! All staff were very friendly and created a welcoming environment. At breakfast, Nancy and Mary Ann had meals ready to go for students as they arrived in line based on the kids' preferences. The meals offered provide great variety and multiple fresh fruit/vegetable options! The cashier station is also set up to encourage a reimbursable meal by providing a fruit bowl at the end of the line in addition to the salad bar at the beginning of the line. Site - Level Commendations: PSD GLOBAL ACADEMY (7198) Despite the limited space, the students are served efficiently. PGA allows students to pre-order their lunch which helps cut down on food waste. Tammie had great rapport with the students and ensured all meals were reimbursable. Site - Level Commendations: PUTNAM ELEMENTARY SCHOOL (7218) Julie, the kitchen manager, did an excellent job ensuring the students had a reimbursable meal. She is knowledgeable on meal pattern requirements and does a great job communicating with the students. Putnam's kitchen was clean and organized. Julie has done a great job managing the kitchen. The salad/vegetable bars at lunch were appealing and provided a colorful variety for the students. Site - Level Commendations: TAVELLI ELEMENTARY SCHOOL (8460) Sponsor and staff were open to suggestions and made recommended changes on-site. The salad bar was colorful and inviting; the students were excited about the variety that the salad bar offers. The site displayed an appealing variety of fruit at the POS station for students to take, which helped students complete their reimbursable meal if needed. Site - Level Commendations: WELLINGTON MIDDLE SCHOOL (9374)

3 Kim and Jill work well together and have open communication with each other. The team work and synergy in the kitchen is evident as they efficiently produce excellent meals for the students. Smarter Lunchroom techniques were observed in a variety of areas throughout the service area to include having the students pass through the salad and fruit bar first, in addition to, placing a bowl of fruit next to the POS. Staff engage with the students and have a great rapport. Review Findings Yes No Sponsor - Level Findings 100 - Certification and Benefit Issuance 100 - Certification and Benefit Issuance 0100 0100 110. The direct certification notification letter used by the SFA does not contain all required information. The letter did not include the Use of Information Statement. 126. Selected applications were incorrectly approved. One student was approved for free meals with no application on file. A written letter from the student's parent was on file which said the student was a household of one, but no income application was on file. SA ensured compliance by adding the Use of Information statement to the direct certification letter. See attachment. SFA called the parent, who then submitted an application online on 12/5/2017. The application was approved for reduced meals and verified by the SA. SA verified that a letter of adverse action was sent to the household on 12/5/2017. 1400 - Food Safety 1400 1402. Documentation is not available to support the exception to purchase nondomestic foods or consideration for the purchase of alternative domestic products as a substitute when the food product ordered is prohibitively costly or sufficient quantity is not available. Non-compliant Items observed: Berries from Mexico, Cucumbers from Mexico, Canned Pears from SA ensured compliance by reviewing the Buy American Exception Worksheet completed by the SFA on-site.

4 South Africa, Canned Peaches from China, Mandarin Oranges from China, Pineapples from Mexico, Tomatoes from Mexico, Garlic Powder from China. 200 - Verification 0200 207. The application(s) selected for review was/were incorrectly verified. One reviewed application was incorrectly verified due to the net income being used instead of the gross income. SFA called household to verify that the income was received monthly, and that military pay types were included based on the eligibility manual guidelines. This application was approved for reduced meals and should have been denied based on high income. SA ensured compliance by correcting the verification approval and changing the student's eligibility to the correct status (paid). SA observed a letter of adverse action (with the correct grace period) that was sent to the household on 12/6/2017. CO-1 0001 The LEA and SFA are not in compliance with the standards set in place by the State Competitive Foods Policy: 1. A food fundraiser (Rootbeer floats at PGA) was being sold during the lunch meal service. 2. Vending machines were on 30 minutes prior to breakfast service and during the school day at Wellington Middle School. For details and Smart Snacks finding, see site-level finding. 3. The water vending machine was on during both meal services at Wellington Middle School. The funds from the vending machine do not accrue back to the food service account; therefore, the water does not comply with the competitive foods policy. Please provide the steps that the LEA and SFA will take to make this correction and ensure future compliance. Include supporting documentation to validate these corrective actions will be maintained, which may include any agreements between the LEA and SFA.

5 Site - Level Findings: PSD GLOBAL ACADEMY (7198) 1100 - Smart Snacks in School 1100 Fundraisers that do not meet the Smart Snack standards must be one of the three exempt fundraisers per school building per school year approved by the SFA. The root beer float fundraiser was not an approved exempt fundraiser and was being held during meal service. SA ensured compliance by having the SFA stop the fundraiser from taking place and explaining that the fundraiser would need to be approved and served outside of the meal service. 400 - Meal Components and Quantities - Lunch 0400 404. There is no signage posted explaining what constitutes a reimbursable meal, including the requirement to select at least ½ cup fruit and/or vegetable. Site - Level Findings: TAVELLI ELEMENTARY SCHOOL (8460) 1400 - Food Safety 1400 1404. The most recent inspection report is not posted in a publicly visible location. Site - Level Findings: WELLINGTON MIDDLE SCHOOL (9374) 1100 - Smart Snacks in School 1100 1105. Foods and beverages sold on the school campus during the school day to students (that are not a part of an exempt fundraiser) do not meet the Smart Snack standards. The vending machine was on prior to breakfast meal service and LEA staff indicated it is on until after breakfast. LEA staff indicated the vending machine is turned off prior to lunch but turned back on right after school. The items in the vending machine sold by the LEA Please provide the steps that the SFA will take to make this correction and ensure future compliance. Include the following supporting documentation with your response: a picture of the signage posted at the serving line. SA ensured compliance by observing the SFA post the most recent health inspection in a publicly visible location. Please provide the steps that the LEA and SFA will take to make this correction and ensure future compliance. If the vending machine will be on during the school day, please provide documentation to support the items sold meet smart snack regulations. If the vending machine will remain off during school hours and until 30 minutes after the end of the school day, please include a detailed plan and agreement

6 1400 - Food Safety 1400 do not meet smart snacks regulations and anything sold from 12am to 30 minutes after the end of the school day must meet smart snacks regulations. At Fossil Ridge High School, the LEA vending machine had one beverage that was out of compliance (Minute Maid sparkling water). 1407. The following storage violations were observed: All food items need to be labeled, in addition to the date, especially items outside of their original package. Food is being stored in the dish area, under the dishwasher. These food items are at risk for cross-contamination as they are near chemicals and water. between the LEA and SFA to validate compliance. SA ensured compliance by having the kitchen manager re-locate the food items away from chemicals and in a safe storage place.
