CHEC-R: Community Health Environment Checklist Public Spaces (PS) & Restaurants (R) SECTION HEADINGS Read the section headings and specific item headings before reading the CHEC Items CHEC ITEMS Read each item carefully. When answering a question, remember to look for the "most accessible" feature within the facility. RULES & GLOSSARY Read the rules to determine how to score an item. If the rule directs you to the glossay, use the picture provided to assit with answerting the question. ANSWER OPTIONS Once you have determined the answer to the question, circle the appropriate response, either "YES," "NO," or, "t Applicable ()." CHEC-PS SECTION I ITEM RULE Distances to Travel to Enter Building 1. Are the distances between personal transportation (i.e. cars) drop off areas and the building as short as possible? Score if accessible parking spots are the closest to accessible route. Include Call-a-Ride and taxis as personal transportation. Score if drop off area is as close as possible to the accessible route. Score if the closest parking spots on accessible route are not designated as Accessible Parking. Estimated Distance: ft. 2. Are the distances between the public transportation drop off areas (e.g., bus stops) and the building as short as possible? Score if the bus stop is within two city blocks and on the accessible route. Score if there are no available bus stops on accessible route within two city blocks. 3. Is there adequate time to cross streets or driveways before cross traffic is given the right of way? Score if from the beginning to the end of the walk cycle of a traffic crossing signal, there are at least 15 seconds for each lane of traffic that an individual must cross. OR Score, if there is no traffic light or walk signal, but there is a pedestrian crossing zone that allows an individual time to cross traffic. Score if the walk signal is not at least 15 seconds long for each lane of traffic. OR Score if there is no walking signal at a traffic crossing signals and no pedestrian crossing zone. Score if there are accessible spaces and at least one space that has an 8 foot parking space and an adjacent 8 foot access aisle. Score if there are no accessible spaces or if the spaces do not have adequate widths and aisles. Score if there is no parking available for the building. If you are unsure if there is parking ask an employee. (For picture see Glossary #1 and #2). Number of lanes: Required time: sec. Actual time: sec. Accessible parking 4. Are there accessible parking spaces with adequate widths and aisles? Total number of spaces: Number of accessible spaces: Width of space(s) ft. Width of access aisle: ft. 5. Are the accessible spaces located closest (or most central) to the accessible entrance or accessible route with minimal traffic to cross in order to enter the building? Score if accessible spaces are closest to the accessible route and most central to the entire group of businesses. Score if the parking spots closest to the accessible route are not accessible parking. OR Score if accessible spots are not central to the entire group of businesses and not on accessible route. Score if there is no parking available for the building. If you are not sure if there is parking for a building, ask an employee. 6. Does the facility have an enforcement procedure to ensure that accessible parking is used by only those who need it? Score if, when management was asked: What would you do and what are your employees to do if a customer reports that an accessible parking space is being used by someone who doesn t need it? he/she answered that the police would be notified immediately of the situation, or that the manager would ask the person to move the car to a regular parking space. Score if management does not respond as indicated above. Score if there is no parking. Level surface 7. Are entrance thresholds even or no more than ¼ inch high? Score if thresholds are even or no more than ¼ of an inch high. Score if thresholds are more than a ¼ of an inch high. (For a picture, see glossary #28). Score if all of the following conditions are met: The surface is free of loose gravel, large cracks, debris, and uneven pavement. The flat surface has a slope of at most 1:20. The width of the surface is about 3 feet wide. Score if all three of these conditions are not met. Score if the slopes of any ramps are no steeper than about 1:12, or one inch of rise for every 12 inches of length. Score if the slope of any ramps is steeper than 1:12. Score if a ramp is not provided. Score if railings are present on both sides and are at a height of about 3 feet above the ramp surface. Score if railings are not present on both sides. OR Score if there are railings, but they are lower than 2.5 feet or higher than 3.5 feet. Score if a ramp is not provided. Score if the ramp is at least 3 feet wide (measure from railing to railing or curb to curb). Score if the ramp is narrower than 3 feet. Score if a ramp is not provided. Height of threshold: in. 8. Does the route to the accessible entrance have a smooth flat surface and is it wide enough? OR Is there a lift or ramp to enter the building? If you answered, which conditions were not met? Slope ratio: If a ramp is provided: 9. Is the ramp not too steep for a person with a mobility device to use? 10. Are there railings on both sides that are reachable from a seated or standing position? Railing height: in. 11. Is the ramp wide enough for a wheelchair to fit through? Width: in. 12. Does the ramp have a resting spot every 30 feet? Score if a ramp has a resting spot every 30 feet. Score if a resting spot is not provided every 30 feet. Score if a ramp is not provided or if the ramp is less than 30 feet long. If you answered,, what is the distance between resting spots? ft. 13. If there are stairs and no ramp at the main entrance, is there an alternative accessible entrance that can be used independently? Is there signage to indicate where the entrance is located? Score if the main entrance is not accessible but there is signage to indicate an alternative accessible entrance. Score if there is no accessible entrance. OR Score if the main entrance is not accessible and there is no signage to indicate an alternative accessible entrance. Score if main entrance is accessible. Curb Cuts 14. Do any curbs on the route to the accessible entrance have curb cuts (i.e., at driveways, parking lots, and any other drop off areas)? Score if the curb cut is level with the street and sidewalk (no more than ¼ rise to get to the sidewalk or street from a curb cut). Score if there are curbs with no curb cuts. OR Score if curb cut to access sidewalk or street is greater than ¼ rise. Score if, in your best judgment, there is not a place where water might collect when you inspect the area in front of and near the curb cut. Score if, in your best judgment, water might collect in the area in front of or near the curb cut. Score if there is no curb cut. Score if curb is unobstructed. Score if debris or blockage prevents you from using the curb cut. Score if there is no curb cut. Score if there is an automatic door at the entrance. OR Score if the door is not automatic, but you are able to open the door using only 2 fingers. Score if the door is not automatic AND you cannot open the door using only 2 fingers. Height of curb cut: in. 15. Do the curb cuts have adequate drainage? 16. Are the curb cuts clear of debris or other temporary blockage? Doors at Entrances 17. Does the door at the entrance have an automatic opener? OR If doors are not automatic, are they lightweight, both entering and exiting the building? Pressure reading: lbs. 18. Is there enough clear space on the latch side of the door to open the door if using a mobility device? Score if there is at least 18 inches of clear space on the latch side of the door (the side with the handle). Score if there is less than 18 inches of clear space on the latch side of the door. Score if there is an automatic door. Score if the doorway at least 32 inches wide. Score if the doorway is less than 32 inches wide. Space on latch side: in. 19. Is the door wide enough to enter using a mobility device? Width of door: in. 20. If the door has an automatic opener, is the control button for the automatic door opener on the latch side of the door? Score if the control button is on the latch side of the door OR it is positioned at a distance away from the door to allow room for the door to swing open. Score if the control button is not on the latch side of the door OR if it is so close to the door that a person in a mobility device has to back up when the door swings open. Score if there is a sliding door. Score if the door stays open for at least 20 seconds. Score if the doors closes before 20 seconds. Score if there are no automatic doors. 21. If the door has an automatic opener, does the automatic door stay open long enough for someone who uses a manual wheelchair (or walks slowly with an assistive device) to go through? If there are 2 doors in a series, Score if BOTH doors stay open long enough to get through the door without getting trapped in between them. Score if one or both doors close before a person can go through. Score if all entrances are accessible. OR Score if inaccessible entrances have signs that indicate the location of the nearest accessible entrance. Score if inaccessible entrances do not have signs indicating the location of the nearest accessible entrances. OR Score if there are no accessible entrances. Time door stays open: sec. Signage for Accessible Paths and Entrances 22. Are all entrances accessible? OR Do all inaccessible entrances have signs indicating the location of the nearest accessible entrance? CHEC-R Section I ARRIVING AT THE RESTAURANT Space for Wheelchair in Reception Area ITEM 1. Is there a reception area? M1 RULE Score if there is a reception area. Score if there is NOT a reception area. Accessible Waiting Area Dining Rooms 2. Is there a clear, wide space for a person using a wheelchair to move around the reception area? M2 3. Are there accessible places to sit that are integrated into the waiting area seating? M3 4. Are alternative dining rooms available? Score if there is a 5-foot circular space for a person using a wheelchair in the reception area. Score if there is not a 5-foot circular space in the reception area. Score if the restaurant does not have a reception area. Score if there are accessible places that are integrated into the waiting area seating. Score if the accessible space is at least 30 inches wide and 32 inches deep. Score if there are no accessible spaces integrated into the waiting area seating. Score if the accessible space is less than 30 inches wide and 32 inches deep. Score if the restaurant does not offer a waiting area. Score if alternative rooms are available. Score if there is only one dining room. Amount of space for wheelchair: in. x in.. Amount of space for seating: in. x in.. Acoustics H3 5. Are there materials that buffer sound? H5 Score if the restaurant has materials that buffer sound (e.g., drapes, carpet, or papered walls). Score if the restaurant does NOT have any materials that buffer sound. CHEC-PS SECTION II Using the Building Walk through the accessible pathway for each of the activities within the building. Determine if there is an accessible route or space and evaluate that space. If there is not accessible designation, assess the general public space. ITEM RULE Doors inside the building (not including restroom doors) 23. Are all doors lightweight, both entering and exiting the door? Score if you can open the door using one or two fingers. Score if you cannot open the door using one or two fingers. Score if there are no doors inside the building. Score if all doorways are at least 32 inches wide. Score if there are doorways less than 32 inches wide. Score if there are no doorways inside the building. Score if there is at least 18 inches of clear space on the latch side (the handle side) of the door, or if there are double doors with clear space to open the doors. Score if there is less than 18 inches of clear space on the latch side of the door. Score if the interior doors are automatic. If #25 was scored, Score if the interior doors have signs indicating the location of accessible pathways. Score if the interior doors do not have signs indicating the accessible pathways. Score if all pathways are accessible. Amount of pressure to open door: lbs. When looking at doors, look only at the doors on the accessible route that visitors to a building would use. 24. Is the door wide enough to enter using a mobility device? Width of doorway: in. You will need to walk through all of the accessible doors in order to score. 25. Is there enough clear space next to the door on the latch side of the door to open the door if using a mobility device? If you answered, how much space is on the latch side of the door? in. 26. Loaner scooters or wheelchairs 27. If appropriate, are loaner scooters or wheelchairs available? Score if loaner scooters or wheelchairs are available. Score if loaner scooters or wheelchairs are not available. Score for small facilities, such as a specialty shop or restaurant, or for a facility that does not have personnel present, such as a park. Signage for accessible elements 28. Are all restrooms accessible? OR Do all inaccessible restrooms have signs indicating the location of the nearest accessible restroom? Score if all restrooms are accessible, or if inaccessible restrooms have signs indicating the location of the nearest accessible restroom. Score if inaccessible restrooms do not have signs indicating the location of the nearest accessible restrooms. Score if there are no restrooms on site. 29. Are all features of a building accessible? OR Are there signs to indicate where all accessible features are located? (if they are different from what is offered to the general public). Examples include elevators, telephones, etc. 30. Is the facility located on a single level? Score if all features of the building are accessible, or if there are signs to indicate where accessible features are located. Score if all features are not accessible and there is no signage to indicate where the accessible features are located. Single level 31. If not, is there an elevator that is large enough to enter and exit with a mobility device? Score if there is an elevator with a door that is at least 36 inches wide, and there is a 5-foot turning radius inside the elevator. Score if there is no elevator, or if the elevator door is less than 36 inches wide or the turning radius is less than 5 feet. Score if the facility is located on a single level. 32. Can controls be reached from a sitting position? Score if the controls for the elevator are between 9 and 54 inches from the floor for side/parallel approach or the controls are between 15 and 48 inches from the floor for forward approach. OR Score if there is a lift that can be operated independently by someone with limited reach using a closed fist. If you are unable to use the lift during the assessment, ask the nearest employee how the lift works. Score if you cannot reach the controls from a seated position. Score if there is no elevator or no lift. Score if the elevator doors remain open for at least 10 seconds. Score if the elevator doors do not stay open for at least 10 seconds. Score if there is no elevator. Score if the doors reopen when someone is in the path of the doors during closure. Score if the doors do not reopen when someone is in the path of the doors during closure. Score if there is no elevator. Height of controls: in. 33. Do the elevator doors remain open long enough for a person with a mobility device to enter and exit? Length of time doors are open: sec. 34. Do the doors reopen if someone does not make it into or out of the elevator car before the doors start to close? Maneuverable spaces Look at all of the public spaces including aisles, community spaces, meeting rooms, checkout counters, etc. Do not include path of travel or restrooms. 35. Are there clear wide spaces in which to maneuver easily when using a mobility device? Score if the accessible route to all public spaces is at least 36 inches wide. Score if the accessible route to all public spaces is less than 36 inches wide. Width of route: in 36. Can a person using a mobility device reach all of the spaces in a facility, maneuvering without having to back out the same way they came in? Score if there is a 5-foot circular or T-shaped space (refer to diagram) for a person using a wheelchair to reverse direction. Score if there is not a 5-foot circular or T-shaped space. Crowding 37. Can a person with a mobility device easily reach all spaces or services without encountering merchandise or people crowding or blocking their path? Score if all aisles and pathways to materials and services are at least 36 inches wide, or if there is an alternative accessible pathway that is clearly marked. Score if aisles and pathways to materials and services are less than 36 inches wide and there is no clearly marked alternative accessible pathway. Score if the floor is a smooth surface. Score if the floor surface is worn, torn, or loose, or has floor mats (more than 1 and excluding entrance mat). Score if the carpeting is low-pile, tightly woven, securely attached along edges and is a solid color. All items must be present to receive a score of. Score if the carpeting is NOT low-pile, tightly woven, securely attached along edges, or is NOT a solid color. Score if the restaurant does NOT have any carpeting. Width of pathway: in. Floor surface 38. Is the floor surface smooth and easy to walk or roll over? V1 39. Is carpeting (if present) low-pile, tightly woven, a solid color, and securely attached along edges? OR if no carpeting is present, is the flooring not worn, torn, or loose? V2 40. Is the floor surface free from clutter and debris? Counters and merchandise 41. Are the counters and merchandise within reach from a seated or standing position? Score if a portion of all counters is no more than 36 inches high or there is space at the side for passing items to customers who have difficulty reaching over any high counters. Score if there is no portion of the counters that is less than 36 inches high or there is no space at the side for passing items to customers. Score if there are no counters or merchandise. Score if there are accessible places to sit (knee spaces at least 27 inches high, 30 inches wide, and 19 inches deep) that are integrated into the regular seating. Score if there are not accessible spaces to sit integrated into the regular seating. Score if the building does not offer seating. Height of counters: in. Accessible places to sit 42. CHEC-R SECTION II USING THE RESTAURANT Accessible Floor Surface ITEM 6. Are steps inside the restaurant (if present) clearly marked with contrast tape or paint on the edge of the step? V3 RULE Score if steps are clearly marked with contrast tape (e.g., striped black and white or yellow and black; solid white or yellow tape on black step) or paint (e.g., white or yellow paint on black step) on the edge of the step. Score if steps are not clearly marked with contrast tape or paint on the edge of the step. Score if the restaurant does not have steps. Lighting in Dining Area 7. Is the dining room well-lit so a customer can see? V5 Score if there is enough lighting to read the menu without difficulty. Score if there is NOT enough light to read the menu without difficulty. 8. Are light sources provided at the tables or does the restaurant provide a flashlight for customer to borrow? -OR- Does the restaurant provide another dining room with more lighting? V6 Score if the tables provide a light source (e.g., light fixture or a flashlight for customers to borrow) at the tables. OR Score if there is another dining room with more lighting. Score if the restaurant does NOT provide a light source at the tables and there is NOT another dining room with more lighting. OR Score if the restaurant only has candles. 9. Do the features of the room enhance the lighting to see sign or read lips easily?
