Rental Agreement Mental Health and Wellness Center Revelle Hall 1210 SW 136 th Street Burien, WA 98166 Welcome to Navos! We are proud to support the community with available public meeting and event space. Requests for use of Revelle Hall can be made by contacting the Executive Assistant, Sarah Elkin at 206-933-7228. Requests for use of rooms inside the Mental Health and Wellness Center (MHWC) may be made by contacting the Administrative Director, Miriam Miyake at 206-257-6964. Eligibility for Use Non-profit agencies, community organizations, public officials, government, city and county agencies; private parties are not eligible. Availability Monday through Friday; 8:00 am to 8:00 pm Fees $100.00 for an event greater than 4 hours. $60.00 for an event less than four hours. Payment can be made either by credit card or check, see page 6. Damages A major credit card is required to reserve the space, and will be held on file for charges which may be incurred due to loss or damage to the property inside Revelle Hall. Requirements for usage: Catering Food & Beverage To fulfill and maintain the integration of recovery in our services at Revelle Hall, and promote the social enterprise philosophy behind the development of Navos Café & Catering, the purchase of our consumer run catering service is required as follows: Events scheduled for less than 2 hours are not required to utilize Navos Catering. Events scheduled for 2 or more hours are required to purchase a beverage and snack service. Events scheduled for 5 or more hours are required to purchase a catered or box lunch service. For groups of 15 people or larger a 15% gratuity will be charged to the final bill. Outside food and beverages are not allowed. Navos Catering requires two weeks (14 days, excluding holidays) advance notice for catering services. Navo s Catering will make every effort to meet catering requests that are made less than two weeks in advance from a scheduled event. However, services cannot be guaranteed. Payment for catering services can be made by check or credit card, and will be due XXXXX Please contact Navos Catering to discuss menu options and any special dietary needs. Changes within 10 business days of the event cannot be guaranteed and are subject to the discretion of the Catering Manager. Events and catering can be cancelled two weeks (14 days, excluding holidays) prior to the scheduled event. The room rental fee will be reimbursed and there will be no catering fees. Events that are canceled less than two weeks (14 days, excluding holidays) prior to the scheduled event will not be fully reimbursed, $20 of the rental fee will be retained. Catering that is canceled less than two weeks (14 days, excluding holidays) prior to the scheduled event will be billed 25% of the catering order. Catering that is canceled one week (7 days, excluding holidays) prior to the scheduled event will be billed 50% of the catering order. Setup & Takedown Navos staff is available for set-up and take down services according to your specifications. Room set-up options are attached, which are dictated by Fire and Building codes associated with maximum occupancies and ingress/egress routes. Occupancies change according to how the space is utilized, maximum room capacity is 120. Available for use are the following: 17 six foot tables 1 six foot catering table 80 chairs (a classroom style set-up can comfortably seat 3 chairs per table which would utilize 51 chairs and 17tables) If we have not pre-approved another set-up arrangement, we require the space remain in its standard configuration. You are not allowed to reconfigure the room, or borrow furnishings from other rooms. Navos will not be liable for any personal injuries, damage to personal property, or damage to the facility resulting from your setup and takedown activities and will hold you responsible for associated damages. Presentation Materials / Decorations Freestanding presentation materials and decorations may be brought into the facility. A minimum of two weeks (14 days, excluding holidays) prior to your event, discuss with our staff what you intend to bring for approval. Please note the following: You are responsible for removing all materials at the conclusion of the event. Revelle Hall is equipped with media options; a projector and screen, sound equipment and microphones. You will need to provide your own laptop for projection of PowerPoint presentations and/or videos. AV support may be available please let us know what your AV needs are at a minimum of two weeks prior to the scheduled event. The use of staples, push pins, nails, tacks, duct or masking, or carpet tape is prohibited. With our prior approval, the use of non-marking masking/painters tape may be acceptable. All tape must be removed at the conclusion of the event. Hanging decorations and materials from light fixtures, in-room equipment, windows, railings, and ceilings is not allowed. We do have some hanging options; please contact our facilities department at 206-799-1268 at a minimum of two weeks prior to the event. Hanging any equipment from doors and windows is not allowed. Standalone projection screens are allowed but cannot block doors, entrances, or exits. June 2013; REV.10/13, REV 7/14-3 -
Fog/smoke machines, pottery kilns, uncontained candles, etc., are not allowed. Additionally, dry ice, rice, birdseed, confetti, and other similar materials are not allowed. If the above conditions are not met to Navos satisfaction, your credit card will be billed for the cost of cleaning and/or repair of the facility according to current hourly rates paid by Navos. Additionally, items that need to be replaced will be billed out at the full replacement cost. Deliveries & Storage You must arrange for deliveries to occur during your reservation period and be on-site to receive them. Navos staff is not authorized to receive and sign for your deliveries and due to space and security concerns; cannot store items for your event. If you plan to have items delivered, discuss the details with our staff at least 14 days prior to your event so we can provide you with the designated delivery areas and access considerations. Parking Off-site parking is available. There is a small parking lot northwest of campus and neighborhood parking on the eastside of Ambaum. Please reference the attached parking map.
