Page 1 of 5 Effective Date: October 1, 2018 Revised from: January 29, 2018 Policy: The child food package shall be customized to meet the client s nutritional/medical needs, food preferences, food intolerances and living circumstances. When tailoring consider cooking and shopping limitations. For example, food packages for homeless clients may include milk issued as and small packages of juice. Food packages for clients, who are homeless or clients in situations where access to water, cooking, refrigeration or storage of WIC foods is limited, may include milk issued as, single serving containers of juice, or peanut butter substituted for eggs. To tailor a food package for these clients, the special authorization window must now be completed to access available substitutions. A Special Issuance Authorization form from a physician is not needed to tailor for these foods. The reason for the special authorization should be specified and if homeless then homeless should also be marked on the demographics window. Children under 2 years of age may not receive peanut butter. Peanut butter is not issued before 2-years of age because it can stick to the roof of the mouth and cause choking. If a caregiver asks about giving peanut butter/peanut products to reduce allergy risk, ask them to discuss this important issue with their physician. The food package shall be customized to prescribe infant formula(s), exempt infant formulas, and medical foods if medically indicated. For example a client may ride the bus to shop, walk to the store or have limited refrigeration. ** Lactose free milk is available in either ½ gallons or, be sure to select the container size available where the client/caregiver shops. A combination of cheese, yogurt or tofu may be substituted for no more than 4 of milk for Food Package IV.
