Construction Q. Can Flaggers use a cell phone? A. Flaggers shall be able to communicate with each other verbally, by means of signals that cannot be mistaken for flagging signals or by two-way radios. Headsets shall not be used. The use of cellular phones while performing flagging duties is prohibited. Reference IDOT Flagger Handbook. Q. During night work who does CM contact? A. After Business Hours Requests 3PM-7AM are addressed in the Tollway Lane Closure Reference Guide on page 30. Tollway operates at 24/7, utilize project contact phone numbers and contact the Dispatch Center. Q. What if Emergency Closures are needed? A. Emergency Lane Closure/Holiday is addressed in the Tollway Lane Closure Reference Guide on page 30. During regular hours use project contact phone numbers. During overnight hours-phone Dispatch Center. Vacation Alternates contacts: Wayde Tabor/ Scott Kapton Q. What if the contractor is running late and lane openings are delayed? A. Emergency extension approval is needed if the closure is being delayed in removal. Follow Emergency Lane Closure Request Procedure on page 30. CM to notify Dispatch Center with reason and time delay estimate. Q. What guidance can be used in construction equipment storage outside the clear zone? A. Reference Tollway Supplemental Specifications Article 701.13. During working hours, all vehicles and/or non-operating equipment and material stockpiles which are parked or stored for 2 hours or less shall be located at least 8 feet from the edge of the nearest moving traffic lane. During non-working hours, or during working hours for periods of more than 2 hours, all vehicles and/or non-operating equipment and material stockpiles shall be parked or stored outside the clear zone or shall be located at least four feet behind man-made or natural barriers which in the opinion of the Engineer serve to fully protect the storage area and not constitute a hazard to motorists. Temporary concrete barrier sections which are installed in conjunction with lane closures will be considered an acceptable means of protection of storage areas, subject to approval of the Engineer. Clear zone distances can be obtained using Table 3.1 form the AASHTO RDG for applicable work zone speed limit. Q. Are performance based designs included in Record Drawings? A. The CM is responsible for creating Record drawings which comprise of as-builts, including shop drawings, performance based items and value engineering items. 1
Environmental Q. What materials are included in CCDD? Are electrical items included? A. Tollway Design Bulletin 13-01 describes the guidelines for the management of clean or demolition debris, uncontaminated soil used as offsite fill and contaminated soil. CCDD include soil, concrete, and asphalt. Q. Run-off events, how is snowfall measured? A. All temporary and permanent erosion and sediment control measures must be inspected, maintained, and repaired as needed to assure effective performance of their intended function. At a minimum, NPDES inspections are required weekly or within 24 hours of a 0.5 precipitation event or greater or equivalent snowfall. Q. Some contracts do not have the CCDD Special Provision included in the contract. How will it be introduced? A. Tollway Environmental Unit will coordinate with the Tollway PM on a project by project basis. Q. Who secures the permitting for Clean Water Act Section 404? A. The Tollway will secure all permitting. CWA Section 404 establishes a program to regulate the discharge of dredged and fill material into waters of the United States, including wetlands. Responsibility for administering and enforcing Section 404 is shared by the U.S. Army Corps of Engineers (USACE) and EPA. Q. Explain the CM role in IEPA Form LPC-662, Source Site Certification by owner or operator for use of uncontaminated soil as fill in a CCDD? A. The CM should follow the following procedure in submitting form LCP-662: 1. Secure the Phase I Env. report and 662 form developed by the DSE 2. Instruct the Contractor to conduct ph sampling, per SP 3. Include ph results on 662 form 4. Submit completed 662 form to Tollway for Chief Engineer signature 5. Once signed, coordinate with Contractor to determine preferred CCDD receiving facility 6. Provide estimated quantity/volume of material to be disposed of at the facility on the form, per the material management plan developed per SP 7. Provide the Contractor s chosen CCDD facility a completed 662 (ph results and Tollway signature), ph report and Phase I Env. report for review Q. Explain the CM role in IEPA Form LPC-663, Uncontaminated soil certification by licensed professional Engineer for use of uncontaminated soil as fill in a CCDD. A. The CM should follow the following procedure in submitting form LCP-662: 1. Secure the Phase II Env. report and 663 form (DSE signed and sealed) developed by the DSE 2. Coordinate with Contractor to determine preferred CCDD receiving facility 3. Provide estimated quantity/volume of material to be disposed of at the facility on the form, per the material management plan developed per SP 4. Provide the Contractor s chosen CCDD facility a completed 663 (DSE signed and sealed) and the Phase II Env. report for review 2
Environmental (cont.) Q. Explain the CM role in IEPA NPDES Permit ILR10, National Pollutant Discharge Elimination System. A. A signed Notice of Intent (NOI) form and Signed S.P. 111 must be submitted to the Tollway Environmental Unit 30 days prior to the start of construction. The Environmental Unit will submit these to IEPA. The NOI must identify a Field Office location for SWPPP viewing. In the event of a discharge, an Incidence of Non-Compliance (ION) must be completed. A signed Notice of Termination (NOT) should be submitted to the Tollway Environmental Unit upon 70% uniform vegetative coverage of disturbed areas. Q. Does the Contractor need to be present when CM is performing inspection for NPDES? A. Yes, the inspection is to be a joint effort in which the CM provides direction to the Contractor, if deficiencies are discovered. Safety Appurtenances Q. Do suppliers of temporary traffic control equipment know what quality standards for temporary traffic control devices are required by the Tollway? A. Suppliers and Contractors are to apply the Tollway s requirements prior to delivery of devices to the jobsite. Reference Tollway QUALITY STANDARD FOR WORK ZONE TRAFFIC CONTROL DEVICES. Q. Is the patching of defects in temporary concrete barrier permitted? A. The Tollway is currently reviewing whether this procedure will be allowed. Q. How are wood blockout certified for required grading? A. IDOT Standard Specifications, Article 1007.06 Steel Plate Beam Guardrail and Wood Guardrail. The posts, rails, and other timber shall be according to the Southern Pine Inspection Bureau or the West Coast Lumber Inspection Bureau. Material certifications from suppliers should be provided. Q. Are guardrail blockouts treated wood? A. Yes, Reference IDOT Standard Specification 1007.12 Preservative Treatment. Preservative treatment shall be according to AASHTO M 133. Q. How is existing guardrail determined to be updated to current standards? A. Tollway MOT Manual Article 6.4.7 (j) describes conditions to remove and reinstall existing guardrail. Q. What is the warranty period for guardrail? A. The Contractor warrants that all work completed under the contract pay items, including all materials and workmanship furnished by the Contractor and subcontractors shall comply with the contract, and that the work shall be free from defects or failures for the period specified after commencement of the warranty period. Guardrail falls under 109.08(b) Tollway Supplemental Specifications Guaranty Against Defective Work for a period of 1 Year. 3
Safety Appurtenances (cont) CONSTRUCTION MANAGER S TRAINING Q. Why does the size of blockouts utilized in Type T6 change from 12 to 8 depth where thrie beam rail is introduced? A. The traffic barrier terminal Type T6 was designed with 8 blocks and is a non-proprietary design. This is a connector terminal that includes an asymmetrical transition section, special posts, block outs, and an end shoe. Q. How do you retrofit a leaveout into pavement? A. Saw cutting of pavement around each post would be required to provide the opening as shown on Standard C1. Q. What manufacturers for temporary energy attenuators does the Tollway allow? A. The Tollway allows the use of any temporary energy attenuator listed on the Illinois Department Transportation, Bureau of Materials and Physical Research (Maintained by Bureau of Safety Engineering) APPROVED LIST OF IMPACT ATTENUATORS, TEMPORARY. All Energy attenuators must meet requirements of the National Cooperative Highway Research Program (NCHRP) 350, TL-3 and be FULLY REDIRECTIVE. Non-redirective attenuators shall not be used on any Tollway facility. Q. What if the maximum span of guardrail per the standard is exceeded when installed in connection with box culverts? A. When guardrail spans a box culvert and the maximum post spacing would be exceeded, then single face barrier wall should utilized across the culvert. The blunt end of the barrier wall would need to be shielded. Guardrail posts shall not be bolted, or attached in any way, to the top of the box culvert. Q. Traffic Barrier Terminals Type T6 and T6B use an asymmetrical transition section, why? A. The height of MGS guardrail and the terminals are constant at 31. This asymmetrical transition section was developed to maintain the required height and transitions the W- beam rail into the thrie beam rail. Q. Why doesn t the Tollway use a Test level 4 or 5 vehicle for energy attenuators? A. Most roadside safety appurtenances are tested at Test Level 3, which requires different crash tests utilizing both a 4400 lb pickup truck and an 1800 lb small car. These two vehicles represent over 90% of the vehicles on the highways in the US. Attenuators must pass 7 different crash tests. Barrier Warrant Process Q. How is an amended BWA processed? Is the existing barrier warrant amended or is a new one created? A. A new barrier warrant for that Area of Concern should be prepared and placed in the appendix of the original document. A note should be placed on the data sheet of the original warrant noting that it has been superseded with a reference to the appendix. The record drawing shall match the new barrier warrant for that location. 4
Punch List Q. What is meant by the identification of Punch List items as extra work or contract items? A. Any items on the punchlist identified as potential extra work should be identified with a disposition. As such, any items deleted from the contract must be identified. Quality Control Q. For the Contractors Quality Plan, can the IDOT inspection checklist be used as a template? A. The Tollway provides a checklist for use; however, supplemental guidelines/checklists should be submitted to the Tollway PM for approval. 5
