X Form as supplied by SBC & Completed by myself.

4 2 All staff have been advised to serve the drinks with clean hands, wearing the supplied uniform. They have been advised to wear non late gloves when preparing fruit. They have also been advised to notify there manager of any illness symptoms immediatly / before attending work. The kegs of Sparkling Wine & Larger (Dlivered by Hills Prospect) are marked on the day of purchase with the date, then dated when opened and then marked with an epiry date. Fruit for cocktails will be purchased from a local supermarket, sliced on the day of use & kept in a a cool bo and disposed of within 48hrs. (if not refriderated (on the bar) this will be replaced & disposed of every 2 hours. Only fruit and Ice for cocktails will be stored, Ice will be frozen and maintained in a freezer with no other foods in the same location. The fruit will be stored in a fridge overnight, again this will be separated and stored in sealed plastic containers if sliced. All drinks and food containers (for fruit) will be clearly marked with epiry dates and checked prior to use. Sliced fruit would be the maimum of 24 hours in a fridge in separate containers & marked with use by and epiry dates (within 48hrs of slicing) Fridge - +4 Degrees / Freezer -5 Daily when in use (displayed on the fridge freezer)

5 We do not prepare any cooked food. Washing up & cleaning table & bar surfaces. Carefully wash cooking equipment, utensils, and food surfaces both before and after food preparation using soap and water hot enough to tolerate or in a dishwasher that uses water at a temperature that kills microorganisms Dettol Antibacterial Spray for surfaces, Beer line cleaner will be used on a monthly basis for the cooler & rinsed with 5L fresh tap water after every use. Fairy washing up Liquid is used for cleaning glasses or cleaned in a dishwasher Non late blue gloves are used when slicing & packaging fruit. Aprons are also used to cover clothing. (No kitchen as this is a mobile bar) X Available from a kettle / thurmos Pumped from a freshwater tank (refilled daily) (At home)

7 The Sparking wine and Larger will be transported in Kegs and cooled by "coolers" at the point of delivery, Ice and Fruit for the cocktails will be transported in "cool" boes and removed from its Packaging / Containers at the time of serving. The temprature of the cool boes will be monitored to keep the temprature at or below +4 degrees, for larger events the products will be transported in cool boes and then stored in Fridge / Freezer that we take with us. No food is displayed, We pour the drinks from the keg & the cocktail miers are kept in there bottles on the bar & out of direct sunlight. N/A N/A We would send the member of staff home / ask them not to attend work until the symptoms have been passed for at least 48hrs.
