1 March, 2017 Lakeshore Band News Spring is finally here----and so is another season of Lakeshore Athletics and Concession Stand! We are in need of parent volunteers to work the various concession stand shifts throughout April and May. Our Concession stand is our biggest fundraiser for the Lakeshore Band program! Please consider volunteering this spring in the Concession stand. It is a great way to support the Lakeshore Band program and Lakeshore Athletics while working a few hours, making new friends and seeing some old, and having fun! 2 FROM THE DESK OF MRS. VK Can you believe it is already time to gear up for next year s marching band? I have several important dates you will want to mark on your calendars as we make plans for next season. Band camp is Aug. 6-11, 2017! The fee for marching band is $450. It will be broken down into two payments of $225. o The first payment is due on April 17, o The second payment is due on May 22, 2017 The fee includes: o Expenses for band camp ($300) o Uniform cleaning/maintenance ($25) o A white t-shirt ($10) o Transportation and State Hotel ($65) o Food/snack/water fee ($50) Note: The food cost covers the cost for Jimmy Johns and/or the main dish (i.e. pizza or hot dogs). We will still ask for volunteers to assist with desserts, sides, and drinks on occasion. We use black drill master shoes for the wind and percussion players and neutral marching/dancing shoes for the colorguard. Shoes cost $45 and we will place the order for these in May. If your student needs shoes, please indicate the size on the registration form and include an additional $45 in your first payment due on April 17, Orders placed after May will include shipping. These forms are due on April 17, 2017, along with the first payment of $225. ALL FORMS ARE AVAILABE IN THE BAND ROOMS AND ON THE SCHOOL WEBSITE UNDER THE HIGH SCHOOL BAND SECTION. Registration Form to gather information about your student and how we can communicate with you. Health Form This form must be signed by the parent or guardian and notarized (we provide that service free of charge to you at the Spring Concerts). Please include a copy of the front and back of your insurance card with your health form. Code of Conduct Form which is your agreement with our student band handbook and agreement with the code of behavior as outlined in the handbook. Ford Field Form which is a legal document granting your student permission to perform at Ford Field for the State Championship.
3 Monday, April 17 Marching Band Registration and Camp Sign-up Current 8 th graders - during your class Current 9 th graders - 7:45am Current 10 th graders -7:30am Current 11 th graders -7:15am On Monday, April 17 please turn in. 1. Your first $225 payment * (checks only: Lakeshore Band Parents) 2. Your registration form. 3. Your health form with a copy of your insurance card. If you don t have insurance please write that on the health form. 4. Your Ford Field Form-legal document authorizing your performance at State Championship. 5. Your Code of Conduct Form- Students/parents need to read the student band handbook (found on the website) before signing this document. Only those students with the 1 st payment and all of the completed paperwork will be allowed to sign up for camp housing on that Monday. Students who forget their payment and forms will be allowed to sign up beginning Tuesday, April 18 for whatever choices remain. It is strongly recommended that they come prepared with forms and payment in hand. Monday, May 22 the 2 nd payment of $225 is due. * Requests for special payment plans and scholarship assistance should be directed to Mrs. VonKoenig at lvonkoenig@lakeshoreps.org by Monday, April 22 nd. Please do not wait to request scholarship assistance as those are granted from a limited funding source on a first come/first serve basis. Notarized Health Forms- We require all of the health forms to be notarized. We offer that service to you free of charge! The health forms that you turn in to us in April will be available at tables before and after the Spring Concerts for both Middle and High School Bands in May. Please stop by the table and confirm your signature with the nurses and they will notarize your form. You can sign up for volunteer spots online! Check out the great opportunities to put your expertise to work! It s a great way to stay up-to-date, support the band, and get to know the kids!
4 PEARL HARBOR PAYMENT SCHEDULE PAYMENTS ARE $250 If your High School student will be traveling to Hawaii to participate in the 76 th anniversary of Pearl Harbor December 3-10, 2017,the schedule for the rest of the payments is included below. Checks are to be payable to Lakeshore Band Parents. June 19 July 24 August 21 September 18 October 23 Give-Back Night Fundraisers in support of the Pearl Harbor Trip: April 12-May 12, Soft Touch Car Wash Pass Sale April 25, 2017, Silver Beach Pizza May 15-May 21, 2017 Give Back Week at Dairy Queen in Stevensville NOTES FROM THE NURSE It is hard to believe that we are thinking of Spring when we have not really even had any Winter yet. But, before long we will be preparing for the Blossom Time Parade and the Memorial Day Parades. With that in mind, please make sure that you have sent me any up-dated health information for your student/s and any new or revised insurance cards. I want to make sure that you know, the information you send about your student s health and their insurance information is held in complete confidence. I do not share this information with anyone other than the band directors and then only if it is important to the safety of your student. The health form book is always in my position and never left unattended when we are traveling. Checks are to be payable to Lakeshore Band Parents A separate check needs to be written for each type of payment. For example, do not combine band clothing payments with state ticket orders. Do not staple check to order forms or other paperwork. Use only blue or black ink when writing checks. The bank s scanner cannot pick up any other color of ink. Did you know that your 6 th -12 th grade student can earn credit to use toward future band camp payments and trips by selling items from our famous meat and cookie dough sale? Did you also know that your participation helps make our band program the huge success that it is? Let s all play a part in the success of our band!! Stock up on your favorite items for the fall and winter months such as pizza squares for an after school snack or a quick dinner or pot roast for cold winter nights. Don t forget to order a spiral sliced honey glazed ham for your holiday meals and cookies and pastry braids for other seasonal celebrations. Check out the variety of quality items on the enclosed forms (or simply print them from the band website) and order to support your student and our amazing bands! Things to keep in mind: Please include the STUDENT S NAME on ALL order forms so that credit can be assigned. Please include a PARENT cell phone number on all forms. Order forms are due with payment by Friday, April 28 to the MS or HS band rooms. NO CASH PLEASE!! Checks made to LAKESHORE BAND PARENTS Credit will not be given for orders which are not picked up. Volunteers working the day of pick up assume that the student (or parent) will be picking up their entire order.
