1 Page 1 of 5 Effective Date: October 1, 2018 Revised from: January 29, 2018 Policy: The child food package shall be customized to meet the client s nutritional/medical needs, food preferences, food intolerances and living circumstances. When tailoring consider cooking and shopping limitations. For example, food packages for homeless clients may include milk issued as and small packages of juice. Food packages for clients, who are homeless or clients in situations where access to water, cooking, refrigeration or storage of WIC foods is limited, may include milk issued as, single serving containers of juice, or peanut butter substituted for eggs. To tailor a food package for these clients, the special authorization window must now be completed to access available substitutions. A Special Issuance Authorization form from a physician is not needed to tailor for these foods. The reason for the special authorization should be specified and if homeless then homeless should also be marked on the demographics window. Children under 2 years of age may not receive peanut butter. Peanut butter is not issued before 2-years of age because it can stick to the roof of the mouth and cause choking. If a caregiver asks about giving peanut butter/peanut products to reduce allergy risk, ask them to discuss this important issue with their physician. The food package shall be customized to prescribe infant formula(s), exempt infant formulas, and medical foods if medically indicated. The base food packages for children are: Basket Cereal Breakfast Cereal 36 oz 36 oz Skim, 0%,½%, 1% Milk 4 gallons Dairy 4 gallons (CPA determines 2% Milk need for if underweight.) Whole Milk 4 gallons Eggs Eggs 1 dozen 1 dozen FVB Fresh Fruit & Vegetables $9.00 $9.00 Juice Check Ready to Drink Juice 2-64 fl oz containers 2-64 fl oz containers

Tailor the base food package, as necessary, to prescribe an age and situation appropriate food package. 3. Base food packages are tailored by selecting the desired food item from each food category basket. 4. The baskets and contents for possible substitution are: Cereal Basket Breakfast cereal 36 ounces 36 ounces Infant cereal Substitute 32 ounces for 36 ounces Dairy Basket Whole Milk* 16 Skim, 0%, ½% or 1% Milk * 16 2% Milk* 16 (allowed if CPA determines need for if underweight) Whole Lactose Free Milk (half gallon) ** Whole Lactose Free Milk (quart) **

Whole Evaporated Milk Fat Free or Lowfat Evaporated Milk Nonfat dry milk Cheese Whole Milk Yogurt Low-Fat or Non-Fat Yogurt Tofu Substitute a 12 oz can for 0.75 quart Substitute a 12 oz can for 0.75 quart Substitute reconstituted quart for quart Substitute 1 pound for 3 up to 1 pound Substitute 32 ounces (1 quart) NA for 1 quart milk. Only 1 quart yogurt per food package is allowed Substitute 32 ounces (1 quart) for 1 quart milk. Only one quart yogurt per food package is allowed. Substitute 1-16 ounce container for 1 quart of milk *Quarts of milk roll up to a base food package of gallons. Milk may be tailored to ½ gallons or for shopping convenience or living circumstances. For example a client may ride the bus to shop, walk to the store or have limited refrigeration. ** Lactose free milk is available in either ½ gallons or, be sure to select the container size available where the client/caregiver shops. A combination of cheese, yogurt or tofu may be substituted for no more than 4 of milk for Food Package IV.
