1 CHISTIANA DELWESFEES APIL 2018 Beste Stalletjiehouer, Aangeheg vind asb die volgende: * Aansoek vorm * `n Brief van die Distriks Munisipaliteit in verband met `equirements for food stalls. * `n Aansoek vorm vir `Certificate for Acceptibility for food Premises. Voltooi asb die vorms en stuur asb so gou as moontlik terug na my per epos of faks. Die vorm sal dan na die Distriks Munisipaliteit gestuur word waarna die gesondheids inspekteer `n sertifikaat sal uitreik gedurende die loop van die fees. Aansoeke wat nie betyds terug ontvang word nie, sal veroorsaak dat u nie toegelaat sal word om besigheid te verrig tydens die fees nie. U gas sertifikaat moet ook asb beskikbaar wees en nie ouers as 24 maande wees nie. Verder herhinner ek dat elke stallejie ten minste van een 4.5 Kg brandblusser moet beskik asook `n brand kombers `fire blanket`. Die diensdatum op die brandblusser mag nie ouer as 12 maande wees nie.
