Receipt of this completed agreement form by the Calgary Health Trust is confirmation of the Sponsor s support of the event as set out above. Upon receipt of the form, the Calgary Health Trust will send an invoice for payment of the sponsorship amount and the Sponsor is to provide such funds to the Calgary Health Trust no later than two weeks prior to the event. 3. As a supporter of the Great Italian Wine Encounter 2014, the Sponsor will be entitled to receive the rights and benefits outlined in the accompanying sponsor benefits document. Please refer to the sponsor benefits document for a detailed listing. These benefits can not be activated until this signed agreement is received by the Calgary Health Trust. Please note that sponsorship fees are non-refundable and not eligible for a tax receipt. 4.
