1 Lake Health Foundation 2017 Team Member Basket Raffle February 1 28 Drawing: March 3, 2017 The Team Member Basket Raffle is up and running! Proceeds from the basket raffle benefit the Employee Assistance Fund. This is the greatest way to help our fellow team members in need. Ticket Prices are as follows: 1 Ticket $1 10 Tickets $5 25 Tickets $10 Please be sure to put your name and phone number on the back of each ticket. If you purchase tickets from a department rep., include the basket number the ticket represents. Baskets will be on display based on the following schedule: Administration Building Wednesday, February 8 th 11:00 am 3:00 pm (Foundation Hallway) Tuesday, February 21 st 11:00 am 3:00 pm Thursday, February 23 rd 11:00 am 3:00 pm TriPoint Medical Center Monday, February 6 th 11:30 am 1:30 pm (Dining Area) 3:00 pm 4:30 pm Wednesday, February 22 nd 11:30 am 1:30 pm 3:00 pm 4:30 pm West Medical Center Tuesday, February 7 th 11:30 am 4:30 pm (Lobby) Monday, February 20 th 11:30 am 4:30 pm If your site is not represented in the above schedule and you would like to purchase tickets, please contact Tina Baucher at or Tina.Baucher@lakehealth.org. A photograph of each basket, along with a detailed description is included on the following pages as well as on the intranet and website. If you would like to sell tickets at your location, please contact the Foundation and we will provide you with the materials you need. All tickets and payment collected must be returned to the Foundation no later than noon on Tuesday, February 28 th. Acceptable forms of payment for raffle tickets are cash, check payable to Lake Health Foundation, payroll deduction (for transactions of $20 or greater and team members only), and credit card (Visa, MasterCard, Discover, and American Express).
