1 2015 USS Schofield DEG/FFG-3 Reunion Information/Registration The 2015 USS Schofield DEG/FFG-3 reunion will be held September at the Holiday Inn Gurnee Convention Center. The hotel is located at 6161 West Grand Ave, Gurnee, IL If driving use I-94 and exit at IL-132 W/GRAND AVE West to first stoplight. The Holiday Inn Gurnee features newly renovated guestrooms with a King or 2 Double beds, and complimentary Wi-Fi access, and Holiday Inn signature Egyptian cotton bedding. Additionally, you ll find an iron and ironing board, alarm clock and personal care and bath items. All guests enjoy complimentary access to the pool, fitness center, and 24-hour business center. The onsite Spice Restaurant and Lounge serves breakfast, lunch and dinner. Check in time is 4:00 PM, check out time is 12:00 Noon. The Holiday Inn Gurnee does not offer complimentary airport shuttle; however there are numerous commercial shuttle services offering transportation to Gurnee. One such company that specializes in individual or small group transportation is American Taxi. You can call American Taxi at or visit their website Mentioning this company does not constitute an endorsement by PRS. The Holiday Inn has its own parking facility and parking is free to hotel guests. Motor Homes/Campers can be accommodated but living in one on hotel property is not allowed. This hotel is perfectly located for this reunion. The Holiday Inn is a non-smoking hotel carrying a steep charge for violators. The information contained herein should answer most of your questions; however, if anything is unclear, please do not hesitate to call our Customer Service Representative at Reservations: All reservations, including lodging, must be made through Premier Reunion Services, LLC. The Holiday Inn will not offer the reunion rate to individual call-ins. Please select one of the two options shown below, or create your own limited agenda. You may fax, mail or phone in your registration. Reservations via fax or phone require the use of Discover, American Express, MASTERCARD or VISA. 2. Payment Schedule: Initial deposit is due on or before August 10, After the initial deposit, you may make up to three installment payments in any amount as long as the full amount is paid by August 21, Cancellations: ALL CANCELLATIONS MUST BE MADE THROUGH PREMIER REUNION SERVICES, LLC. YOU MUST OBTAIN A CANCELLATION NUMBER FROM PREMIER REUNION SERVICES, LLC. CALLING THE HOTEL AND CANCELLING YOUR RESERVATION IS NOT SUFFICIENT. For cancellations received on or before September 4, 2015 there is a $50.00 cancellation fee plus the cost of any activities that have already been paid for, or for which guaranteed numbers have been provided to the vendor. There will be NO REFUNDS for any reason - for cancellation requests received after September 4, For your protection and so you do not lose your investment, Trip Cancellation Insurance is highly recommended. Three nights lodging (Thurs, Fri, Sat); Three breakfasts (Fri, Sat, Sun) Friday dinner buffet; Saturday night banquet; Thursday through Saturday use of hospitality room (stocked with soft drinks, coffee, and snacks); Great Lakes, IL city tag; 2015 reunion memory book; roster of attendees; all applicable taxes, gratuities (except tour guide & driver); bartender, set-up fees; registration fee; and the full time services of a Premier Reunion Services, LLC manager. Cost of this option is only $ per couple; $ per single. A $ per person deposit must accompany your reservation. Up to three installment payments in any amount may be made as long as the full amount is paid by August 21, USS Schofield DEG/FFG Reunion Itinerary Thursday September 10, :00 Noon: Registration Begins and the Hospitality Room opens stocked with coffee, soft drinks, and snacks. Because of liability considerations, private alcoholic beverages cannot be brought into the hospitality room. The hospitality room will close during the welcome reception. 5:00-6:00 PM: Welcome reception begins. This is the first planned function. After a few welcome and admin announcements you will enjoy a build-your-own-deli sandwich. If you like a beer (or something else) with your hors d oeuvres there will be a cash bar. Dinner, if you need it, is on your own tonight. Cost of the reception is included in the price of Option A only. If you cannot participate in the reception, you may opt out. See the Registration Form, Adjustment Opportunities Block, delete Thursday s reception. Friday September 11, :30 8:30 AM: Breakfast will be in the hotel restaurant with coupons provided. Due to the early morning departure for Great Lakes the Holiday Inn will make provisions to insure there is ample time for breakfast to be served to everyone prior to boarding the motor coach. Be sure to check the restaurant hours when you arrive at the reunion. Cost of breakfast is included in the room rate so you may not opt out. 7:15 AM: Load em up; 7:30 AM Move em out to the Great Lakes Naval Training Center for the opportunity to witness a boot graduation. Please be aware that seating at the boot graduation is limited. Parents and relatives get first priority. The remaining seats IF ANY - are allocated on a first-come basis. Schofield is first in line (we got our request in early) so our chances are excellent, but not guaranteed. Final approval will not be given until six weeks (or less prior to the requested dates. In the unlikely event the visit is not approved, the tour will be cancelled and balance dues adjusted accordingly OR you will have a tour of the base without seeing the graduation. In order to be allowed on the base as a tour group, an advance list of names must be sent to the Base Security at least 72 hour prior to arrival. We are told there is no exception to this policy, so there can be no last minute sign ups. You must register for the visit prior to the cutoff date found in the registration material. At the gate you must also show a government issued form of identification. Expect delays at the gate while a security check is made. Persons under the age of 18, except with youth groups, will not be allowed on base. Following the graduation there will be a riding tour of the base, consistent with security conditions. You will not be able to get off the bus at any time during the tour. Photos are not allowed except at the graduation. A stop will be made for lunch at the Port O Call restaurant (formerly the Officers Club) on base. Lunch is on your own. You will Premier Reunion Services, LLC PO Box Hickory NC Page 2

3 return to the hotel around 2:00 PM. To be sure there is no misunderstanding, today s transportation will be a school bus. The cost is less than half of a 55 passenger motor coach. This has been approved by the group leadership. A guide is not needed since it is just a short drive and the Navy will provide an on base escort. There is a minimum of 25 persons for the tour to take place. Less than 25 the tour will be cancelled or the cost adjusted to make up the difference. Since this is an optional trip, the cost is not included in either of the options. You must register separately, See the Registration Form, Adjustment Opportunities Block, and add Friday s tour. 6:00 PM: Social Hour with cash bar. 7:00 PM: Dinner is served. Tonight s dinner is a gourmet buffet followed by some self-entertainment. Cost of dinner is included in both options. If you cannot participate in the dinner, you may opt out. See the Registration Form, Adjustment Opportunities Block, delete Friday s dinner. Saturday September 12, :30 8:30 AM: Breakfast will be in the hotel restaurant with coupons provided. Cost of breakfast is included in the room rate so you may not opt out. 8:45 AM: Load up; 9:00 Depart for Discover Chicago. First stop will be the Museum of Science and Industry where you will be fascinated with all of the exhibits; but the famed German U-505 maybe the most interesting to you. On June 4, 1944, a German submarine known as U-505 was prowling off the coast of West Africa on a hunt for American and Allied ships, when depth charges from the USS Chatelain blasted the dreaded U-boat out of hiding. It was the end of a violent run for U-505, which had terrorized the Atlantic Ocean as part of a massive U-boat campaign that almost altered the outcome of World War II. The Museum of Science and Industry invites you to step inside the real U-505 the only German submarine in the United States, and, now, a national memorial to the 55,000 American sailors who gave their lives on the high seas in WWI and WWII. Your admission ticket allows you to explore this historic boat. Other exhibits and there are too many to name each one include the Silver Streak Pioneer The Coal Mine, Colleen Moore s Fairy Castle, Earth Revealed, Farm Technology, Ships through the Ages, and dozens of others. Some exhibits require an additional admission fee (on your own) if you choose to visit them. You are sure to be thrilled with every aspect of this great museum. When you leave there for the Navy Pier your guide will point out some more of Chicago s sites and sights. The Navy Pier originally opened as a shipping and recreational facility in Located on Lake Michigan, it has served many purposes throughout its rich history and currently encompasses more than fifty acres of parks, gardens, shops, restaurants, family attractions and exhibition facilities and is the top leisure destination in the Midwest, drawing nearly nine million visitors annually. You can have lunch on your own upon arrival at the Navy Pier or at the Museum before leaving (so long as you make it to the bus on time). There is a minimum of 25 passengers for this tour to go. Less than 25 the tour will be cancelled or the price adjusted to cover the difference. Today we will have a regular motor coach and a guide. You will return to the hotel about 3:00 PM. Individual and/or couples photos will be taken for inclusion in the 2015 Reunion Memory Book. As soon as possible after the reunion, the memory book will be sent to everyone who registers for Option A or B; or anyone selecting the Individualized Option who places an order for the memory book. Be sure to have your photo taken! 7:00 PM: The USS Schofield Banquet. Enjoy your last night in a more formal setting. Coat and tie recommended for the gentlemen and corresponding dress for the ladies. If you can still wear a complete and correct uniform, please do so. There will be open seating, sit where you like, with whom you like. The memorial service will be a part of the banquet program. Cost of the banquet is included in both options. If you cannot participate in the dinner, you may opt out. See the Registration Form, Adjustment Opportunities Block, delete Saturday s banquet. Sunday September 13, :00 AM: Breakfast will be in the hotel restaurant with coupons provided. Unless you have an early flight or must hit the road early join everyone for breakfast.
