1 A Summit DD initiative, in partnership with BlickClinic FALL 2015 SESSION SEPTEMBER-OCTOBER 2015 The COLLEGE FOR LIVING provides opportunities throughout the year for individuals with developmental disabilities to participate in and explore the many local resources available in the community. Funded through Summit DD, this program offers citizens with disabilities a wide variety of activities to socialize, explore hobbies, or discover new interests. As the primary force in the community dedicated to inclusion, Summit DD believes that communities are stronger when they include everyone with or without a disability. Discover more about Summit DD at BLICKCLINIC is a private, nonprofit, outpatient clinic that began in 1969 with a group of parents of children with developmental disabilities and a few volunteer professionals. Together, they developed the Clinic into a single, comprehensive source of diagnostic evaluation, treatment, and support group services to persons with developmental disabilities. [LIMIT OF 15] - September 4 5:00-8:30 p.m. [REGISTRATION DEADLINE: August 24 or when class limit is reached.] Location: Blick Clinic, 2641 West Market Street, Akron (the new building on Market Street in Fairlawn, between N. Revere Road and Fairlawn Town Centre, on the same side as the Town Centre). Instructors: Tami Mastrojohn and Kalee Dotson ( ) Come join us for a tropical luau! We ll be enjoying games, having a contest for the best Hawaiian outfit, hula dancing, decorating sugar cookies, eating tropical food (pineapple and assorted fruits), chicken, soft drinks and punch, and a whole lot more. Please note dietary restrictions on the registration form. #2 DINNER AND A MOVIE [LIMIT OF 15] - September 9 5:30-9:30 p.m. [REGISTRATION DEADLINE: September 3 or when class limit is reached.] Location: Drop Off: Steak N Shake, 4000 Medina Road, Montrose Pick Up: 9:15-9:30 p.m. (depending on time movie ends) Regal Cinema, 4020 Medina Road, Montrose Instructors: Linda Consolo and Charlotte Morris ( ) Accompanying Staff: Should bring $ for dinner and movie ticket EMERGENCY CONTACT PHONE NUMBERS (to be used only on day of event): and This event will be held at Steak 'N Shake in Montrose with the movie at Regal Cinema. We will dine at the Steak 'N Shake and then take a short walk to the Regal Cinemas. Movie choices will be announced the day of the class. Participants should dress appropriately for the weather and be able to walk the short distance from Steak ''N Shake to the movie theater. Instructors are not permitted to drive participants from the restaurant to the movie. If a participant is not able to walk independently, then transportation arrangements should be made for him/her. The dinner and movie ticket are included in the price of the class. Participants should bring additional money if they wish to purchase extra items from the dinner menu or concession stand. Please note dietary restrictions on the registration form. #3 CANVAS ART PAINTING & DINNER [LIMIT OF 10] - September 11 5:30-8:30 p.m. [REGISTRATION DEADLINE: September 4 or when class limit is reached.] Location: Blick Clinic, 640 West Market Street, Akron Instructors: Kalee Dotsun ( ) and Kristen Olsen You do not have to be an artist to create something amazing and have a fantastic time doing it! Join us for an evening of canvas art painting, along with dinner and dessert. All supplies and instructions will be provided by Kristen Olsen, MA, AT (Founder of Artful Expeditions). Participants will have the choice of popular patterns or simply create their own masterpiece! Dinner will consist of create-your-own-taco bar, soft drink, and dessert. NOTE: This class and class #18 are the same activity. If you sign up for this class, you CANNOT sign up for class # 18, this will allow other to have an opportunity to participate in this activity. Please note dietary restrictions on the registration form. #4 PORTAGE LAKES KIWANIS CENTER COOKOUT [LIMIT OF 10] - September 12 12:00-2:00 p.m. [REGISTRATION DEADLINE: August 30 or when class limit is reached.] Location: Kiwanis Club of Portage Lakes, 725 Portage Lakes Drive, Akron Instructors: Paula Sokol and Laura Townes (from the Summit DD Board ) Accompanying Staff: No $ needed, but must have a head count for food. Hamburgers, hotdogs, covered dishes, desert, and beverages will be provided. There will also be karaoke and the game Corn Hole. Please note dietary restrictions on the registration form. #5 CREATIVE CANVAS [LIMIT OF 20] - September 16 4:45-8:45 p.m. [REGISTRATION DEADLINE: September 2 or when class limit is reached.] Location: Barberton Center, 501 W. Hopocan Avenue, Barberton Instructors: Sara Williams and Jean Murphy (Summit DD Board) We will be calling our inner artist. Participants should wear clothes that they can paint in or a paint shirt to wear. We will be making some of canvas art while listening to music, and learning that you do not have to be an artist to be artistic! Pizza and soft drinks will be served for dinner. 3 #6 SUMMIT MALL SHOPPING September 17 5:30-8:30 p.m. [REGISTRATION DEADLINE: September 10] Location: Summit Mall Food Court, 3265 West Market Street, Akron Instructors: Charlotte Morris and Jeniele Hughes ( ) Accompanying Staff: Should bring their own $ for this activity. EMERGENCY CONTACT PHONE NUMBER (to be used only on day of event): Wear your walking shoes so we can shop until we drop! Bring your spending money as we will visit your favorite stores. We will start out at the Food Court for a light dinner (included in the class price). Please note dietary restrictions on the registration form. #7 OKTOBERFEST [LIMIT OF 12] - September 20 11:30 a.m. to 3:30 p.m. [REGISTRATION DEADLINE: September 13 or when class limit is reached.] Location: Meet at Metropolitan Popcorn, 2164 Front Street, Cuyahoga Falls Instructors: Mary Jo Goldman ( ) and Jessica Cole Accompanying Staff: Must bring own $ for food purchase. This will be a fun-filled day of food and music, and if you want, dancing. There will be numerous vendors there to visit. Each participant will be allotted $10 to purchase food from any of the vendors. Participants can also bring extra money if they wish to purchase more food or other items from the vendors. Please note dietary restrictions on the registration form. #8 SPAGHETTI AND BINGO NIGHT [LIMIT OF 18] - September 23 5:00-9:00 p.m. [REGISTRATION DEADLINE: September 16 or when class limit is reached.] Location: Spaghetti Warehouse, 520 South Main Street, Akron Instructors: Jeniele Hughes and Charlotte Morris ( ) Accompanying Staff: Need to bring own $ if dining with the participant Come join us for a fun-filled night of bingo and prizes! We will also enjoy a spaghetti dinner together. Please note dietary restrictions on the registration form. #9 MANICURE AND MOVIE NIGHT [LIMIT OF 12] - September 25 5:00-8:30 p.m. [REGISTRATION DEADLINE: September 14 or when class limit is reached.] Location: Blick Clinic, 640 West Market Street, Akron Instructors: Tami Mastrojohn and Kalee Dotsun ( ) This is a chick-flick night! Ladies come and get pampered with a manicure, have some yummy snacks, and then enjoy a movie. We ll have lots of fun! Please note dietary restrictions on the registration form. #10 M.A.P.S. AIR MUSEUM TOUR [LIMIT OF 15] - September 26 11:00 a.m. to 3 p.m. [REGISTRATION DEADLINE: September 18 or when class limit is reached.] Location: 2260 International Pkwy, North Canton Instructors: David Wood and Ebony Thompson ( ) Accompanying Staff: Individuals supporting CFL participants do so at no cost, but lunch is not provided, and a small donation to the museum is encouraged. This is a visit to the Military Aviation Preservation Society (MAPS) Air Museum in North Canton. MAPS Air Museum is a nonprofit organization dedicated to educating people about the history of aviation and its impact upon society. We will enjoy a customized group tour of the museum, a provided picnic lunch among the display aircraft, time to explore on our own, and the opportunity to visit the museum gift shop for souvenirs. Please note dietary restrictions on the registration form. #11 CAMPFIRE COOKING [LIMIT OF 20] - September 30 5:00-8:30 p.m. [REGISTRATION DEADLINE: September 20 or when class limit is reached.] Location: Wingfoot Park, 993 Goodyear Blvd, Mogadore Instructors: Sara Schismenos and Sarah McCoy (Summit DD Board) Accompanying Staff: No $ needed, but must have a head count for food. Meet us at Wingfoot Park, where we will show you the proper way to build a campfire, go over campfire safety, tell campfire stories, and cook our dinner over the fire! 4 #12 DJ DANCE [LIMIT OF 10] - October 1 6:30-8:00 p.m. [REGISTRATION DEADLINE: September 15 or when class limit is reached.] Location: St. Francis de Sales Parish (school gym), 4009 Manchester Road, Akron Instructors: Paula Sokol and Laura Townes (from the Summit DD Board ) DANCE, DANCE, DANCE! Put on your dancing shoes and come and enjoy an evening of music and dance to a DJ s jams. The dance will take place at St. Francis de Sales parish s school gym. Cookies and beverages will be provided for participants. Please note dietary restrictions on the registration form. #13 BOWLING NIGHT [LIMIT OF 20] - October 2 5:30-8:30 p.m. [REGISTRATION DEADLINE: September 25 or when class limit is reached]. Location: Bill White s Akron Lanes, 2911 East Waterloo Road, Akron Instructors: Tami Mastrojohn and Kalee Dotson ( ) Accompanying Staff: No $ needed Bowl the night away! Have some fun bowling and eating some yummy pizza at Bill White s Akron Lanes. We hope to see you there. Please note dietary restrictions on the registration form. #14 HARTVILLE MARKET & KITCHEN [LIMIT OF 15] - October 3 9:30 a.m. to 1:30 p.m. [REGISTRATION DEADLINE: September 28 or when class limit is reached.] Location: Drop Off: Front entrance of Hartville Market Place-Flea Market, 1289 Edison Street NW (aka Rt 619), Hartville Pick Up: Front entrance to Hartville Kitchen restaurant, 1015 Edison Street NW (aka Rt 619), Hartville Instructors: Linda Consolo and Charlotte Morris ( ) Accompanying Staff: Should bring $ for the meal at the restaurant. EMERGENCY CONTACT PHONE NUMBERS (to be used only on day of event): and Join us as we shop our way through Hartville Market Place looking for that special 'treasure' for yourself! Participants are responsible for bringing their own spending money and handling their money for shopping at the Market Place. After shopping till we drop, we'll relax and enjoy a delicious lunch at Hartville Kitchen. Lunch at the restaurant is included in the cost of this class. After lunch, participants will shop in the gift shop and bakery, located at the Hartville Kitchen. Participants should be on time for this event as the group cannot be held up from shopping in order to wait on late arrivals. If an emergency arises and you are unable to attend, or will be late, please phone the emergency contact phone number prior to 9:30 a.m. so that arrangements can be made to meet late arrivals. Please note dietary restrictions on the registration form. #15 BOO AT THE ZOO [LIMIT OF 12] - October 11 3:00-6:00 p.m. [REGISTRATION DEADLINE: September 10 or when class limit is reached.] Location: Akron Zoo, 500 Edgewood Avenue, Akron Instructors: Tami Mastrojohn and Heather Campbell-Wilson ( ) Accompanying Staff: Should pay the class price. Come and join us at the Akron Zoo for a great time during "Boo at the Zoo"! We'll be going to different areas in the zoo collecting treats and seeing the animals too. Participants should be able to walk independently as we'll be walking throughout the entire zoo. You can come dressed up in your Halloween Costume!! (NOTE: Per Akron Zoo Policy: Costumes should not include items such as guns or any weapon-like object. Accompanying staff/family members and instructors CANNOT wear face paint or masks. Participants are allowed to wear masks or face paint.) You'll receive a fun bag that day to collect your goodies, so you don't have to bring one. Please inform instructors of dietary restrictions prior to the class as other meal choices cannot be substituted. #16 SUBWAY AND MOVIE NIGHT [LIMIT OF 12] - October 13 5:30-9:30 p.m. [REGISTRATION DEADLINE: October 6 or when class limit is reached.] Location: Drop-off: Cinemark Theater, 2925 State Road (Portage Crossings), Cuyahoga Falls Pick-up approximately 9:30 p.m., depending on time movie ends. Instructors: Linda Consolo ( ) and Sheryle Schiavoni Accompanying Staff: Should bring $ for dinner and movie ticket EMERGENCY CONTACT PHONE NUMBERS (to be used only on day of event): and Join us for a Subway Dinner and Movie Night at Cinemark Theatre, located at Portage Crossings on State Road. This activity includes a boxed Subway dinner including sub sandwich, chips, and beverage. Please select the type of sub on form with your reservation. (If no choice of sandwich is marked on the registration form, a turkey sub will be selected for you.) There will be a choice of two or three movies, which will be announced the day of the event. Cost of this event includes your Subway dinner, movie ticket and a special popcorn and beverage combo. Participants may bring additional money to purchase extra items from the concession stand. At time of drop off participants and their driver will be informed of the end times of movies. Please check in with an instructor so you know the pickup time. Please note dietary restrictions on the registration form. #18 FALL FESTIVAL AT SZALAY S FARM [LIMIT OF 12] - October 17 11:00 a.m. to 1:30 p.m. [REGISTRATION DEADLINE: October 9 or when class limit is reached.] Location: Szalay s Sweet Corn Farm & Market, 4563 Riverview Road, Peninsula (Drop off will be at the table area.) Instructors: Linda Consolo ( ) and Sheryle Schiavoni Accompanying Staff: Must bring own $ the day of the event for admission and food. EMERGENCY CONTACT PHONE NUMBERS (to be used only on day of event or late or unable to attend): Join us for fun in the fall as we work our way through the corn maze at Szalay's Sweet Corn Farm and Market. Sure to be a good time as we guide our way in small groups from start to finish through the tall corn! You must be able to walk on your own through the maze to attend this event (wheelchairs are also permitted in the maze). It should take approximately minutes to find our way through the maze. Afterwards we will sit at their tables & swings and enjoy a meal consisting of a Deli Hot Dog or a Turkey Burger, roasted corn, and a choice of lemonade, sweetened with sugar or Splenda/Equal or sweet tea. We'll sit and swing while enjoying the company of your College for Living friends. Please note, no substitutions can be made on food but attendees can bring extra money to purchase other food items. Please call only if you will be late or unable to attend that day so we not waiting. Please inform instructors of dietary restrictions prior to the class as other meal choices cannot be substituted. #19 PUMKIN CARVING [LIMIT OF 10] - October 26 5:00-9:00 p.m. [REGISTRATION DEADLINE: October 19 or when class limit is reached.] Location: Tallmadge Center, 630 North Avenue, Tallmadge Instructors: Sara Schismenos and Sarah McCroy (Summit DD Board) We will carving pumpkins and learning about the history of pumpkin-carving and how it got started. 6 REGISTRATION FOR FALL 2015 CLASSES Class size may be limited at the discretion of the instructor, but a minimum of 3 participants per class is needed. The class participant is responsible for their own transportation. College for Living classes are opened only to Summit County residents who are eligible for Summit DD Board services and are at least 16 years of age. In the event of class cancellation or extraordinary circumstances, vouchers will be issued (no cash refunds will be given). Vouchers will be issued on a case-by-case basis and only with approval from the College for Living Committee. You must contact Mike Smeltz, Committee Chair, 24 hours before the class in order to be considered for a voucher. NOTE: Please note dietary restrictions below.
