3 Thank you for joining us tonight. Our goal is to make sure you have a GREAT time while supporting a cause that is important to all of us - eliminating dental infection and pain. The Seattle-King County Dental Foundation Board of Directors and the 2013 Event Planning Committee are delighted you are here. We want to make sure you have a lot of fun tonight so we won t be having silent auctions - just some wonderful wine tasting and great social time. We are guaranteeing that the Live Auction will be done by no later than 8:30 pm. THEN, we can all let our hair down and gamble! We have a smaller number of more selective and unique auction items for which you may bid this year. Because we aren t offering silent auction items, we hope you will give generously during the fund-a-need section of the program. Remember: these gifts are 100% tax deductible. 4 Award-winning glass artist, Janusz Pozniak, has created our lovely centerpieces this evening. His works have been exhibited in the American Craft Museum and the Corning Museum of Glass among other venues. He has worked with Dale Chihuly and many other well-known artists. You may bid on any of the centerpieces on any of the tables. There are several single pieces with starting bids of $100, four sets of two glass pieces starting at $150, and one set that contains three pieces, starting at $200.

Looking for a unique experience to offer your friends, employees or colleagues?! Look no further. Siren Song Wines will provide a wine tasting for up to 25 people. You may hold this tasting at your home, office or some other location. The winery will provide the wine, stemware and expertise. The cheese and crackers as well as the good time had by all is on you! 7 Each offering luminosity and Venetian delicacy, this pair of Reticello Goblets hails from glass artist Dante Marioni s personal collection. Vetro a reticello is a type of internal decoration in which glass bubbles are trapped within a net pattern, a challenging technique that results in a stunning net effect in design and visual texture. Goblets have been a particular focus for Marioni s blown glass work, which is strongly influenced in its form and cane work by the Venetian tradition, yet has a distinctive style all its own Marioni studied glassblowing at Pilchuck School and Penland School of Crafts and has exhibited in major glass galleries and group exhibitions at museums such the Corning Museum of Glass, the Chrysler Museum, the New Zealand National Museum of Art, the Smithsonian Institute and the Yokohama Museum of Art. This fun package is ideally suited for our great summer days! Start you day off with an excursion in one of Seattle s very own hot tub boats. Yes, you read that right you and five of your guests hop into a hot tub that you can then motor around Lake Union, viewing the sites as you go. Then, we will provide a catered lunch delivered to you there. But, WAIT, there s more! Included in this package is a hotel stay for two in downtown Seattle at the Washington Athletic Club. We will also throw in snacks for your hot tub cruise and an extra basket of wine for the rest of your day, as well as a $50 gift certificate for the Seastar Restaurant and Raw Bar in the South Lake Union neighborhood. Such fun! Hotel certificate is available after May 2013 and is subject to availability. Here s some information about some of the programs that your generosity is supporting tonight. Dental Care for Dialysis Patients Since November 2009, Society members have donated over $200,000 in dental care to patients awaiting life-saving kidney transplants. In order to receive a lifesaving kidney transplant, dialysis patients have to be completely free of infection including oral disease. Thanks to the generosity of our dentist volunteers, FOURTEEN patients have received transplants. Swedish Community Specialty Clinic (SCSC) In September 2011, we opened a free clinic that offers complex extractions for patients referred from the community dental safety net. Partnering with several organizations, this dental clinic is located at Swedish Hospital. Designed to treat patients that community clinics cannot, this vital service saves lives. It also saves money as it keeps patients out of hospital emergency rooms a much more costly option. Our first full year of service was in We saw 570 patients, treated 2,163 teeth and provided $731,000 in donated care. The Clinic hopes to be open five (5) days a week soon as the need is great. PROVAIL s Dental Clinic We provided critical funds for dental supplies for their Johnny Johnson Memorial Clinic which treats adults with disabilities many of whom face significant challenges in order to access dental care. With cuts to Medicaid, many PROVAIL patients would have to go without care if the Dental Clinic did not operate. The Seattle-King County Dental Foundation and Dental Society are reducing barriers to accessing dental care. Won t you join us by donating this evening and considering volunteering for these efforts?

8 You and two of your guests can golf with Dr. David Minahan at the Inglewood Golf Club, one of the oldest golf courses in the Northwest. He will treat you to practice balls, provide a cart and host lunch in their architecturally marvelous Clubhouse built in The second part of your package includes joining Dr. Michael Spektor for a foursome of golf at the Glendale Golf Club, located moments from downtown Bellevue. After you play on this course which has hosted 11 State Opens, you will join Dr. Spektor for a celebratory lunch. 11 Our mission is simple, and the need is urgent. The Seattle-King County Dental Foundation seeks to proactively eliminate dental pain and infection among our neighbors. With the cuts to adult dental funding from the State, access to dental care has become even more challenging. As a dental community, we are working on two programs that will help the most vulnerable among us, and the dollars you donate this evening will help fund them. We partnered with Swedish Medical Center, Project Access Northwest and several others to build three (3) dental operatories in the Swedish Community Specialty Clinic. Our dream was simple: provide free complex oral extractions for patients that our dental safety net could not treat. In 2009, we began working with the Northwest Kidney Center where we provide free dental care to dialysis patients. In order to receive life-saving kidney transplants, the patients must first be cleared of all infection--including dental infection. Since this program started, fourteen dialysis patients have received a kidney transplant after being cleared of dental infection and many more are cleared and waiting for an appropriate match. Your contribution to the Fund a Need helps support these partnerships and is 100% tax deductible. We hope you will contribute to these very important causes. 14 Internationally known Northwest Native artist Preston Singletary's work has been shown throughout the world. Now, this gorgeous print, the Eagle and the Killer Whale, can be yours. The strong lines and bright colors are distinctive and reflect the traditional folklore of the Tlingit heritage. The Tlingit have lived for centuries on the southeastern coast of Alaska, and Singletary's work explores the history of this Native group. Signed by the artist, it is print number 56 of 100 and was created in Dimensions (framed) are approximately 13 by 31.5 inches. One Direction is one of the hottest groups touring this summer.
