3 General Information & Policies 3 All prices are subject to change without notice; however, we will guarantee that prices will not increase more than 10% from the date you make your deposit. In addition, functions that are completely planned will not be subject to increase within 90 days of the date of function. A service charge of 22% and WI sales tax of 5.5% will be added to all food and beverage charges (total of 28.8%). A convenience fee of 2% will be added to the total amount of any bill paid by a non member for all events if payment is by credit card. A pre-arranged menu is required for all parties of 13 people or more. For private parties of 12 people or less, menu service will be available. Please call the Club to make menu and other function arrangements at least three (3) weeks in advance of the event. A single entrée selection is strongly recommended for all functions of 100 people or more, but we will provide a vegetarian selection if requested. If two choices plus a vegetarian are offered to parties of 100 or more people, a $1.00 per person surcharge will be added to the bill. Vegan or gluten free selections are always available, and do not count towards the two entrée limit. Confirmation on meal functions, as to the number of guests to be served and exact counts for each entree, must be made no later than 72 hours (3 days) prior to the function. The counts for functions on Tuesdays or Wednesdays are due the preceding Thursday a.m. All charges will be based on the guaranteed count plus any last minute changes after the guarantee has been given. Color-coded place cards, when more than one entrée selection is being served, must be provided by the host and approved by Blackhawk Country Club in advance. Evening or all-day functions reserving the entire dining room will require a $1, deposit. This deposit will be credited to the function's total charges, and will be refundable if cancellation occurs before 270 days of the reserved date. If cancellation occurs after that time and the Club is able to rebook the facility, the deposit will be returned. The Club is generally closed on Mondays; however, with approval, the Club will open for select functions. Any special equipment that is requested, which the club needs to rent, will be billed at the rental charge plus 10% handling. Hosts are responsible for any and all damages done to the Club during their function. Cost of repairs and cleanup will be charged to their account. Signs and Displays - A minimal amount of pre-approved display material is permitted in the club foyer or dining room; no nails or tacks please; no tape on the paper wallpaper. Any damages caused by signs and displays will be charged. The throwing of confetti or rice is prohibited; however, birdseed or flower petals are permissible. Only drip-less or canister candles may be used with candelabras. All pyrotechnics must be pre-approved by Management. No pyrotechnics allowed in Clubhouse or on any green space Smoking is not allowed in any area inside the Club or any of the outside patio areas. Guests may smoke on the cart path (below the patio). No entertainment may be scheduled to perform later than 12:00 a.m. After the entertainment has completed their performance, one hour will be allowed to take down and remove equipment from the club. The sponsor or host of the group will be charged for additional time taken to remove their equipment at the rate of $50.00 per hour. Last call at the bar will be no later than 11:45 p.m. Liquor will not be served past 12:00 a.m. (midnight). All decorations put up for a non-wedding function must be taken down before departure. If the Club is required to take down decorations, a $30.00 fee will be assessed. No food prepared by the club, which remains after a function, may be taken out of the club. No food or beverage may be brought in to the Club without advance approval. 3

Additional colored napkins may be rented at a cost of $0.25 per napkin. Votive Candles: The club will provide up to three candles per table at no cost. Additional candles can be provided at the cost of $0.50 per candle. Mirrors: Use of 12 round mirrors is available at a cost of $3.00 per mirror Chairs and tables are provided at no charge with the exception of silent auction and display tables, which are charged at the rate of $5.00 per table. There is no fee for cake tables, place card tables or gift tables. AV Equipment Wireless microphone, podium, and indoor public address are provided at no charge Outdoor public address Screen (8 high with a 80 x 48 viewable area) TV (40 flat screen)/dvd/vhs cart Projector HD Projector/Duel HD Projectors 75.00/ Club Laptop Wireless internet connection is available at no charge Use of Flip Chart Outside Food and Beverage Service of any outside bakery, including wedding cakes and cupcakes, will incur a $1.95 per person charge.
