2 LUXURY SUITE INFORMATION Jesse Scaglion, your personal representative, is available to assist you with your food and beverage selections if you have any questions or special requests. Jesse can be reached Monday through Friday, 8:30am 5:00pm or via at any time. Phone: (O) (M) PLACING YOUR ORDER Please fill out one of our order forms that will be provided to you by Jesse Scaglion or your account rep. These forms must be completely filled out including, suite number, contact information, event date and arrival time. We encourage you to appoint one person to place all suite and beverage orders to ensure accuracy and avoid duplication. To make your suite visit as effortless as possible, we ask that all food and beverage orders, as well as any special requests, be placed at least 72 hours prior to your event; menus will be provided to you shortly after you book your event. Any order or changes made after the deadline will be taken care of by your suite attendant upon your arrival. 3 LUXURY SUITE INFORMATION DELIVERY OF ADVANCE ORDERS In order to provide you and your guests with the finest service, we will deliver your advanced order an hour prior to the game time unless special arrangements have been made. Our service staff will assist you with final touches and preparation. While in your Suite, an attendant will be assigned to service you throughout the duration of your event. Those services include setting up and maintaining the cleanliness of your suite. The attendants will also be available to great your guests, deliver your food and beverage as well as provide personal attention to you and your guests. We ask all advanced orders be placed 72 hours prior to the event. DAY OF GAME ORDERING If you are unable to place an advanced order, we welcome you to order from our game day menu when you arrive. Simply contact your suite attendant to learn more about this menu and to place all orders. Due to availability and preparations, specific items will not be available for day of game ordering. Game day orders will not be ready to be served immediately but we will make sure that you and your guests receive your orders in a timely fashion. ALCOHOLIC BEVERAGES It is the responsibility of our Suite attendees to monitor and control the consumption of alcoholic beverages within the suite. State Law prohibits alcoholic beverages from entering or leaving the facility grounds. Minors under the age of 21 are not prohibited to consume alcoholic beverages at any time. If you are leaving your suite please place beverages in a plastic cup which we have provided for you. CANCELATION / RAIN OUTS In the event you must cancel your order, please notify us immediately. Orders canceled within 12 hours of the event will be charged 100% of the food and beverage order. In the event of a rain out, there will be no need for a cancelation notice. However, if guests are in the suite consuming food or beverage, and the game SUITE is canceled due to rain, the entire order MENU will be charged to the final bill. SUITE ACCESS Suites are open 60 minutes prior to all regular Legends games unless otherwise posted for special events or circumstances. Guests may remain in the Suites 60 minutes after the conclusion of the game. Guests are not permitted to reenter the ballpark through any gate once they have exited.

4 PAYMENT INFORMATION METHODS OF PAYMENT Following are the payment options for suite holders. Please review these options to determine which best suits your suite's needs. CREDIT CARD ON FILE A credit card is provided and kept on file to be used for food and beverage charges. Near the conclusion of the event, the main contact will be required to sign a customer summary, eliminating the need to present the card again. We accept: American Express, Visa and MasterCard. ADVANCE PAYMENT For this option, you will be provided with a total of all charges based upon the Advance order. All advance payments (cash, credit card, and certified check) must be received at least 72 hours prior to the game. Please advise your guests that they will be responsible for any additional orders over and above the initial suite order. The suite attendant will be required to ask for payment at the time the order is placed. PAYMENT AT CONCLUSION OF THE GAME Please advise your guests that they will be responsible for all food and beverage charges incurred during the game. Suite attendants are required to ask for a credit card at the beginning of the game for any food and beverage orders. All charges will be posted to this credit card during the game and a signature will be required at the end of the game. TAX AND GRATUTIY There is a 6% sales tax that will be applied to all food and beverage orders per the State of Kentucky. Gratuities are at the sole discretion of suite holders and/or guests.
