BIG GAME TABLE PACKAGES The Foundation Room Lounge will open on February 4th at 1:30 pm for admission and VIP Lounge Packages. All packages are subject to tax and gratuity.

All VIP Packages include entrance into the lounge, reserved VIP seating area for the Big Game viewing, and a food & beverage table minimum. BIG GAME TABLE PACKAGES The Foundation Room Lounge will open on February 4th at 1:30 pm for admission and VIP Lounge Packages. All packages are subject to tax and gratuity. Other Member discounts and gift certificates do not apply. VIP Packages and Table Minimums are available to accommodate varied party sizes. Additional guests may be added to any table reservation for the pricing structure listed below and all package prices include admission. All seating in the Lounge, Private Rooms, Dining Side Patio and Lounge Side Patio are reserved for this service. Foundation Room Members receive 10% off any Big Game package. End Zone Package $2, Includes your own private space for your group to enjoy. This package includes complimentary access for up to eighteen (18) guests, food & beverage minimum. '72 Dolphins Package $2, Includes the very finest in table service the Foundation Room has to offer accompanied with the premier table location in the venue. This package includes complimentary access for up to twelve (12) guests, food & beverage minimum. '85 Bears Package $1, Includes exclusive access to your group within one of our private hideaways. This package includes complimentary access for up to eight (8) guests, food & beverage minimum. '62 Packers Package $1, Includes a personal TV living room area with complimentary access for up to six (6) guests, food & beverage minimum. '91 Redskins Package $ Includes a personal TV living room area with complimentary access with plush seating for up to five (5) guests, food & beverage minimum. '89 49ers Package $ Includes complimentary access for up to four (4) guests, Big Game viewing, food and beverage minimum.

4 Location: Foundation Room is located atop Mandalay Bay at 3950 S. Las Vegas Blvd, Las Vegas, Nevada Deposit Policy: In order to reserve a space, we must be in receipt of a credit card authorization form and a signed agreement from you upon receipt of this package. If a credit card is used as a deposit, the credit card will be held and charged the balance of your table on February 5th. Your VIP Package is not confirmed until the full deposit and a signed agreement is received by this office and is subject to cancellation without notice or consideration. Cancellation Policy: Cancellation of your event within 72 hours of the scheduled time will result in forfeiture of your reservation. 100% applicable charges will be charged. If you have supplied a credit card for your deposit, the deposit amount will be applied to your credit card upon confirmation. All deposits become the sole property of the restaurant and will be forfeited upon cancellation for goods and services rendered in good faith. Check In Policy: Foundation Room lounge will open at 1:30 pm on Sunday, February 4, Special Dietary Requests: All special dietary requests must be addressed prior to the completion of this contract. Foundation Room will do everything reasonable and customary to accommodate dietary requests and needs. Dress Code Policy: In the nature of the sports celebration, Foundation Room will be flexible with its normal dress code for guests attending this event. Show your team pride! Miscellaneous: Must be 21 or older to attend. The host is responsible for any and all damage to the premises incurred by his/her guests. Foundation room does not allow guests to bring outside food and beverage, per policy, into the restaurant without expressed written consent of the Director of Foundation Room Operations. This includes, but is not limited to, wine, champagne, liquor, food prepared or in its raw state, desserts, cakes, etc. Foundation Room and House of Blues reserve the right to inspect and control all private functions. Foundation Room and House of Blues cannot assume responsibility for personal property or equipment brought into the function area. Personal effects and equipment should be removed from the premises at the end of the event. Tax and Gratuity: In addition to the package minimum pricing, 20% gratuity and 8.25% tax will be applied to all food and beverage purchases. Authorization: I have read the above agreement, restaurant policies and procedures printed in this agreement. I agree to the terms and conditions mentioned above. This booking will remain tentative and subject to cancellation by the restaurant until this contract is signed by the organization (you) and received by the restaurant (us) along with a 50% deposit. I further understand that if I choose to cancel my event within a period of 72 hours prior to the scheduled time I will forfeit my 50% deposit to the restaurant for goods and services rendered in good faith. Furthermore, I do not hold the restaurant liable for the cancellation of my event either by the restaurant or myself. I release and indemnify Foundation Room and its staff for any and all loses due to a cancellation on my part. I understand and agree to the terms and conditions of this agreement and enter into it in good faith with Foundation Room. I understand and accept that I am liable for the total cost of the reservation booked that I have agreed to, and that I am liable for full payment of the guarantee, as well as any and all charges above the guarantee, as stated in the guarantee policy. I understand that I am liable for any and all taxes and gratuities associated with this event. By the card holder signing above, you agree to all terms listed in the contract. Host is required to sign only as an acknowledgement of event.
