Special Diets in Care Home Catering By Helen Blunn BSc RD apetito Dietitian Many care homes have a variety of special dietary needs for their residents that they need to meet at each mealtime. Whether this is gluten free, vegetarian or a texture-modified meal, the National Association of Care Catering (NACC) and the Care Quality Commission (CQC) advise that meal service offer an extensive choice of dishes and special diet options to promote a pleasant and dignified dining experience. We work hard to meet these needs through our wide range of care homes meals and desserts. Each dish is tasty, appealing and nutritious while meeting CQC and the BDA Digest standards for care catering; as well as suitability guidelines for most special diets. This guide will help you select from our range of mouth-watering meals and desserts. We have defined our suitability criteria for each of the main diets at the back of this brochure. However, as every person's needs are different and can change, we recommend that each care home works with residents' dietitians for planning menus and monitoring. To help, we can provide you with a complete nutritional breakdown of our products. You can order meals in a variety of tray s, meaning you can exactly match the number of to your requirements. 4 Preparing your menu cycle We know planning menus can be a difficult task. That s why we ll appoint a Client Development Manager (CDM) who is dedicated to helping you prepare varied, nutritious and appealing menus for everyone in your care home. Your CDM will take the time to learn about your needs and residents preferences. They will then help prepare four-week menu cycles packed with tasty dishes. If you wish, they can even arrange a tasting evening so your residents and their relatives can sample the range of food on offer. At the end of the four-week cycle, your CDM will check the system is running well and enhance the service if necessary. Our support doesn t end there. We ll review your menus every four months to make sure they re delivering the right blend of delicious dishes to your residents. Alongside our main meals and desserts, there s also a tempting range of tea-time options. You ll find more details of these on page 3. Your CDM will help prepare varied, nutritious and appealing menus for everyone in your care home / Pork in Cider Gravy We work closely with all our homes to plan menus and make sure that all necessary nutritional requirements are met. Not only do we offer a wide choice of large portion meals and desserts, we also provide a large range of individual meals to reduce wastage and make it easy to cater for residents with specific dietary requirements. We also offer a menu planning system, Nutriplan, which manages your residents needs and ensures that they receive a good level of nutrition. The system gives you the reassurance that your menus meet or even exceed the CQC* and NACC nutritional standards, and provides you with instant proof in the event of an inspection. * CSSIW and care inspectorate 02

5 List A tasty selection of tea-time favourites With apetito, you ll find there s much more to our menus than main meals and desserts. We also offer a wide choice of tea-time favourites that eliminate the need to prepare evening food and give your residents dishes they re sure to love " Premium Sausage Roll Come tea-time, everyone deserves tasty food that delights their tastebuds. That s why we ve worked hard to create a selection of tea-time classics that are sure to tempt your residents. In addition to our existing range, we have a number of tasty pastry items, perfect for tea, supper or snacks. So, whatever the preference, there s always a delicious and nutritious dish that will satisfy their appetites for the evening ahead. Nothing goes better with a cup of tea than a nice slice of cake and it s a great way of encouraging residents to drink more whilst helping to boost their daily calorie intake. There are four delicious loaf cakes in our range, and all are perfect for an afternoon snack or as a supper time treat. Each cake serves ten hungry people and can be baked straight from raw batter in your oven. There are four tasty varieties choose from: Sultana, Chocolate Chip, Carrot and Madeira. 6 NEW RANGE Fork-mashable Cauliflower Cheese and Broccoli Bake introducing our fork-mashable meals; the latest addition to our texture-modified range Everyone deserves to enjoy tasty meals. However, people who cannot chew, swallow and digest foods properly have traditionally lacked access to attractive and nutritious dining choices. That is, until now. Our 19 new Category E fork-mashable main meals complement our current range of Category C (Thick Purée) and Category D (Pre-mashed) texture-modified options perfectly, helping you to meet the standards recommended in the Dysphagia Diet Food Texture Descriptors. The descriptors were developed by the National Patient Safety Agency (NPSA) Dysphagia Expert Reference Group, and state that all care settings must ensure the provision of Category C and Category E meals. The descriptors state that any fluid, gravy, sauce or custard in or on food must be thick i.e. a light disposable plastic teaspoon would stand upright if the head was fully, but just covered. national descriptors for texture modification in adults: B Thin Purée Dysphagic Diet C Thick Purée Dysphagic Diet D Pre-mashed Dysphagic Diet E Fork-mashable Dysphagic Diet Swallowing difficulties mean that people with dysphagia can face challenges to eat and drink safely. We ve worked hard to ensure that our texture-modified meals not only meet the national texture descriptors but are still tempting and nutritious for your residents. 7 List Why Category E Fork-mashable? Fork-mashable meals play an important role in the dysphagia journey. For degenerative conditions such as Parkinsons or dementia, they are the first experience for residents moving to a texture-modified diet. For residents recovering from strokes or trauma, fork-mashable meals signify the last step on their journey, before resuming a normal diet. It is crucial that these meals resemble normal food, for a variety of reasons both physical and emotional. Accepting and adapting to the need for a texture-modified diet is emotionally challenging. The fact that our meals look like normal food is of immense value in this difficult transition time. For both groups, and particularly those returning to a normal diet, it is also important that the meals challenge residents to work the muscles associated with chewing and swallowing. For this reason, we have also included boiled and sautéed potatoes in the range - rather than exclusively using mashed potato - to help exercise the chewing and swallowing muscles. For those recovering, it also provides some much-needed variety Fork-mashable Beef in Gravy What s different about our fork-mashable range? Appearances can be deceptive - ensuring the correct consistency and delivering the specific texture-descriptor guidelines whilst developing these meals posed far more of a challenge than we had anticipated. For example, ours is the only pre-prepared range to offer sliced meats. To attain the correct level of tenderness, we had to source new meat suppliers to provide the approved stamps of meat recommended by the descriptors, as well as extend cooking times to guarantee tenderness. As these meals are less processed than our Thick Purée and Pre-Mashed options, it was also more difficult than we anticipated to fortify the range to the extent we consider desirable for an audience that is likely to be nutritionally vulnerable, whilst preserving the look of a traditional meal. feedback from the experts We ve tested our meals extensively with Speech and Language Therapists to ensure that they conform to the National Descriptors. On their recommendation, we have elected to exclude any pasta dishes - which could become floppy or crunchy upon cooking. We have also excluded high-risk ingredients such as peas, beans and peppers, which have husks or skin, which multiple textures can pose a serious threat to those with dysphagia. what is dysphagia? Dysphagia describes a range of swallowing and chewing difficulties. It can make it difficult to take in enough calories and fluids to nourish the body and can lead to serious medical problems. The condition most commonly occurs after a stroke or other nervous system disorders including dementia, MS or Parkinsons, though it can also be a side effect of throat cancers and treatments involving the mouth, neck or oesophagus. 05

Suitable for multiple meal occasions such as breakfast, lunch, suppertime and snacks, the category C sandwiches range includes: Hot Tuna Sandwich Hot Bacon Sandwich Hot Cheese Sandwich Category C Thick Purée Meals Our category C meals are of a completely smooth and thick consistency, moulded for a natural appearance, and hold their shape after cooking. There are 24 options in the range. Category D Pre-mashed Meals Our category D meals are moist, with a uniform particle. They have some texture variation and are ideal for residents with swallowing and chewing problems. Each meal has been designed with particular care to provide appetising colour combinations, and fantastic flavours. There are 19 dishes in the range. Category E Fork-mashable Meals Our range of category E fork-mashable dishes features 19 main meals, seven of which are vegetarian, and all are suitable for diabetics an offering unparalleled elsewhere in the marketplace. The range also carries six gluten free options. The meal has been specifically developed to meet minimum standards across all care settings for calories and fortification. As with all our products, the range goes through extensive quality testing by our Quality Assurance department, assessing taste, texture and appearance. However, as this is such a specialist product where giving the consumer the wrong texture can potentially prove fatal - we have also introduced additional quality standards in the kitchen to ensure each meal is guaranteed to meet the consistency of the category E descriptor every time, for your ongoing peace-of-mind. Category C Thick Purée Cold Desserts We also carry a range of four cold desserts for the warmer months, or as a lighter dessert option. Category C Thick Purée Hot Desserts Our category C hot desserts are an industry-first, providing residents with access to hot British classics, such as Apple Pie. The texture is smooth and the desserts are layered to retain each distinct flavour, such as apple, pastry and custard, for an unparalleled dining experience. There are 10 desserts to choose from. 38 Dietary s Rationale for Large-Portion Special Diet Coding: D Diabetic Residents with diabetes are advised to try to maintain normal blood sugar levels and achieve a healthy by eating a balanced diet low in fat, salt and sugar. However, there s no reason why they can t still enjoy delicious, wholesome meals. All our soups and main meals are suitable for people with diabetes when eaten as part of a healthy balanced diet. It is not necessary for people with diabetes to have a completely sugar free diet. Moderate amounts of sugar are generally allowed as part of a healthy diet. We have a range of individual diabetic desserts available, many of which are accompanied by low sugar custard. Each individual diabetic dessert contains a small amount of added sugar (no more than 8g per portion). We use a small amount of artificial sweeteners (including aspartame and acesulfame potassium) in some desserts including custards and sweet sauces. Custard, sweetened with an artificial sweetener is available in large portion and 3 portion packs suitable for people with diabetes. LF Low Fat Dietary Fat is an important source of energy and vitamins, particularly for older or under residents with a small appetite. For this reason it is wise to ensure any plans for reducing fat intake are both appropriate and necessary. Some residents who are watching their or have heart problems may request a low fat diet which will typically contain no more than 70g of fat (20g saturated fat) per day for women, and 90g of fat (no more than 30g saturated fat) per day for men. Many dishes in our range may be suitable for these residents. A breakdown of calorie or fat content of the meals can be provided on request. New EU regulations for food labelling mean the term low fat can only be applied to dishes which provide no more than 3g of fat per 100g so we have highlighted these in this section. These have been selected to help the residents choose dishes to aid loss as part of a low calorie plan. Other dishes in the range may also be suitable provided the total calorie level intake is restricted to less than about calories per day. Please consult a healthcare professional if you need further advice choosing meals suitable for loss. We have used the following criteria to large portion products as suitable for reducing diets: Soups with 75 calories or less per portion Main course dishes with 200 calories (kcals) or less per portion Accompaniment: all starchy foods containing 200 calories (kcals) or less per portion All vegetables. Some of our main course dishes containing pasta or potato topping provide less than 400 calories (kcals). These can be d as reducing if residents are advised on the menu not to select an additional starchy accompaniment. LS Low Salt Some salt (or sodium chloride) is important for the flavouring of food. However high intakes are linked with high blood pressure. As a result we have worked hard to reduce the salt content across our entire range of meals in recent years whilst retaining the flavour residents enjoy. New EU regulations for food labelling mean the term low salt can only be applied to dishes which provide no more than 0.3g of salt (0.12g sodium) per 100g and so we have highlighted these in this section. Please note: Unnecessarily strict salt restrictions should be avoided unless medically required since older people may have taste impairment and salt enhances the flavour of food. Occasionally some residents with heart, kidney or liver problems may need a more restricted salt diet in which case the residents dietitian should be consulted to provide more guidance about suitability of dishes for an individual. To aid this we can provide a complete breakdown of salt content for all our products. 36
