Around here we run loud, late and a little crazy. Just the way Mama likes it! Mama Loco Albany is a funky neighbourhood joint located on the exterior of Albany Westfield Shopping Mall on the North Shore. Our love of delicious modern Mexican food combined with a love for tequila and good music, all add up to an unforgettable venue which makes for fantastic functions. 4 FUNCTION INFORMATION PEOPLE/PAYMENT We offer free venue hire for our function areas, however food and beverage minimum spends do apply. If in the scenario that a minimum spend is not met, the difference will be charged through as venue hire. How the minimum spend is met, is at the discretion of the function organiser. Restaurant bookings do not require a minimum spend, but we do require a minimum of 10 people for all set menus. No deposits are required for area bookings, however credit card details will be taken when a booking is made. Any no shows, cancellations or guest number reductions less than 72 hours prior to the function will be charged for at Mama Loco s discretion. Menu selections and dietary requirements must be confirmed at least one week prior to the function. For Private Hire of Mama Loco, a 20% deposit will be required upfront and a minimum spend will apply. JUST A QUICK NOTE If on the day, more people show up than the amount booked for, each additional person will be charged for at the same rate as the rest of the party. If fewer show up, the full amount will still be charged for as all the food will be already sorted for the event. BAR ACCOUNTS You can set up a bar tab for your guests at a credit limit decided by you. Once that limit has been reached you can choose to add further funds or your guests can then pay as they go. As the account holder, you can specify the beverage options available to guests on the account. We can also accommodate tabs which subsidise your guests drinks. Let us know if you would like more info. FOOD FROM THIS FUNCTION PACK The whole group is to be on the same price structure. All of the food in this function pack is to be pre-ordered. Menu selections, dietary requirements and final numbers are to be confirmed at least one week prior to booking date. If pre-ordering does not suit your function, we can always provide you with our main menu for you to look over. ONE SIZE DOES NOT FIT ALL If you ve got ideas not covered in this pack, just talk to us and we ll see what we can do to make it happen. 4

We can source live entertainment for your group if you require it VARIOUS ENTERTAINMENT OPTIONS - POA Talk to us about your function requirements and we will set it up. DECORATIONS - POA Talk to us about any theming that you would like for your function and we can do all the hard work for you. PIÑATAS - POA When in Mexico, do as the locals do. Any accounts/items not paid for, loss, damages or cancellations will be charged to the credit card details provided on this form. 9

10 BOOKING T&C'S FINAL NUMBERS Bookings up to 20 people: Subject to the provisions set out below a signed booking form must be provided within 5 days of tentative booking in order to secure booking. Bookings over 20 people: Subject to the provisions set out below, a credit card authorization form must be provided and returned along with a signed booking form within 5 days of tentative booking in order to secure booking. Confirmation no less than 7 working days prior to the event date is required for all menu options located in this function pack. No less than 7 working days prior to your booking date, final guest numbers, final menu selections (and dietary requirements) are to be provided. The final number of guests provided at this stage is the minimum number that will be charged for on the function account. Management will do its best to allocate requested areas for your event but no guarantee is given. Should fewer numbers arrive, the booked area may be relocated to accommodate for other patrons. GUARANTEED NUMBERS - Minimum numbers for catering, seating and beverage required no less than 7 working days prior to your event. You will be required to pay for the minimum advised numbers or the actual numbers on the day of the event, whichever is the greater. MINIMUM SPEND - In some instances a minimum spend amount may be required to secure the venue or any of its areas on premium dates. If the minimum spend is not achieved, the balance is to be paid at the conclusion of the event. Management reserves the right to release the booking if these terms are not met. Credit card details provided to Mama Loco on the booking form will be held as security and will be charged for any loss, damages and unpaid accounts. If credit card details are not given or available a bond may be required at an amount determined by Management to secure the booking. The bond would be held as security for any loss, damages and unpaid accounts. FOOD AND BEVERAGE - No food or beverages may be brought onto the premises by you or your guests unless the prior consent of management has been obtained. Any beverages in excess of amount pre-arranged with Mama Loco must be secured and paid for with a valid credit card at the time or at the conclusion of your event. No persons under the age of 18 will be served alcohol. BAR TABS - You can set up a bar tab for your guests at a credit limit decided by you. The credit card to which the bar tab is to be charged to must be provided on the day of the function and will be held at the bar until the account is signed off and paid for. DRESS CODE - Tidy dress is required. Please inform your guests of the dress code before the event to avoid non-admission. If your function has a dress up theme please advise staff prior to your event. LOSS / DAMAGE - Mama Loco is not responsible for any loss or damage to any guests or clients property before, during or after the event. You agree to indemnify Mama Loco for any breakages, thefts, damages or extraordinary cleaning requirements caused by you, the attendees or subcontractors and shall pay those costs within 7 days of the event. PAYMENT DETAILS - All final accounts settled at the conclusion of function. There will be no accounts run or invoices sent, unless by prior arrangement with Mama Loco. Any unpaid accounts from the function will be charged to the credit card details provided. All prices shown include GST and are subject to change without notice. CANCELLATIONS - Credit card details provided to Mama Loco on the booking form will be held as security and will be charged in the event of any loss, damages, unpaid accounts or cancellations (as outlined). If credit card details are not given (or available), a bond to the value of $250 will be required to secure the booking. The bond would be held as security in the event of any loss, damages, unpaid accounts or cancellations (as outlined). If none of the above occurs, the $250 bond will be reimbursed after the function. In the event a booking is cancelled less than 2 weeks prior to the event (or 4 weeks during 16 November 1 January) - there will be a $250 CANCELLATION FEE charged to the specified credit card or the $250 bond will be retained. HEALTH AND SAFETY - You are responsible for the conduct of all guests present. You agree to indemnify Mama Loco against any claims which may arise from injury or damages from the use of any subcontractor you employ. In accordance with the Health and Safety in Employment Act 1992 and Mama Loco Health and Safety Policy, clients, visitors and contractors must obey all reasonable instructions and signs whilst on site, to ensure that their actions do not create hazards to people or property. All Fire and Emergency exits are to be kept clear at all times. On hearing an alarm please leave the building by the nearest Fire Exit, assemble at the area designated on the Fire Notice at the Lounge Exit Door and await further instructions. Smoking is not allowed in any indoor area. Signs, placards and advertising are not to be nailed, taped, stapled or velcroed to any surface. Similarly the above are not to be hung from suspended ceilings. The repair of any damage to walls, doors, ceilings etc. will incur an additional cost. LICENCE RESTRICTIONS - Mama Loco operates under a SUPERVISED LICENCE, therefore no person under the age of 18 is permitted on premise unless accompanied by their parent or a court appointed legal guardian. After 10pm all persons under the age of 18 are required to vacate the premises. HOST RESPONSIBILITY - It is illegal in New Zealand to promote intoxication and/or allow intoxicated persons on the premises. Mama Loco are responsible for creating a safe environment which complies with the Sale and Supply of Alcohol Act 2012 or any subsequent applicable legislation, and stand to receive a large fine and suffer temporary licence suspension if found in breach of our legal responsibilities. Therefore all staff have been informed to follow the below policy: 1. To detect early signs of intoxication and encourage persons to consume water and food. 2. To inform the host of the function of such intoxicated persons. 3. To cease providing alcohol to such intoxicated persons. 4. To remove intoxicated persons from the premises. We expect all our guests to conduct themselves in an orderly and proper manner at all times. If guests spoken to do not adhere to our requests, then we will ask and insist that they leave the premise immediately. If, for any reason, Mama Loco is unable to meet its obligations in respect of any bookings, we reserve the right to cancel any such booking without liability and will refund any deposits made.
